Estudio experimental de los efectos de la aspirina y dipiridamol en la agregación plaquetaria

Abstract

El llamado dilema de la aspirina consiste en que no existe un acuerdo unánime sobre el régimen de dosificación que debe emplearse para esta terapéutica antiagregante. Este fármaco produce acetilación irreversible e inactivación de la ciclooxigenasa plaquetaria, con lo cual se produce una inhibición de la sintesis de txa2, agente proagregante y vasoconstrictor, pero además inhibe la de pgi2, agente antiagregante y vasodilatador, efecto este ultimo que deberiamos obviar. Existe un equilibrio especifico entre estos dos productos derivados del ácido araquidonico en el control de la función plaquetaria, la terapéutica ideal debería inhibir la sintesis de txa2 sin afectar la producción de pgi2. En la primera fase de este trabajo se estudia un amplio rango posológico, tratando de evaluar la relacion dosis-respuesta en la accion antagregante plaq. El dipiridamol, farmaco antiagregante plaquetario, es un inhibidor de la enzima fosfodiesterasa y puede ejercer su acción potenciando el efecto de la pgi2. El trabajo pretende en una segunda fase, objetivar el efecto de la interacción dp+ aas sobre la agregación plaquetaria, usando una única dosis de dipiridamol (1 mg-kg) asociada a las dosis anteriormente reseñadas de aspirina. Los resultados confirman la posibilidad de que dosis bajas de aas inhiban la agregación plaquetaria reduciendo la sintesis de txa2 sin afectar la producción de pgi2. El dp muestra acción antiagregante adicional con determinadas dosis de aas.
