Microorganismos y caries dental

Abstract

El estudio de la caries dental es un ámbito tan complejo que se ha clasificado como una disciplina científica en sí, la etiopatogenia de esta enfermedad se basa en cuatro factores fundamentales: las bacterias, el diente, la dieta y el tiempo. Esta enfermedad se caracteriza por una pérdida de mineral óseo debido a un incremento del ácido, causado por la actividad metabólica bacteriana. En la boca existen bacterias comensales que, con el aporte de grandes cantidades de azúcares, provocan una disbiosis convirtiéndose en patógenas. Hay relación entre varios géneros microbianos y la aparición de caries, pero no existe una única especie bacteriana causante de esta enfermedad, por esto, se entiende como una alteración del biofilm que ya está presente de manera natural en estado de salud. Los géneros Streptococcus y Lactobacillus, entre otros, se relacionan con la colonicación inicial de las piezas dentarias, posteriormente se produce agregación de otros géneros como Actinomyces y Fusobacterium que se anclan a esta matriz previamente formada. La capacidad para la formación de moléculas extracelulares como glucanos, tolerar el ambiente ácido o fermentar gran variedad de azúcares con producción de elevadas cantidades de ácidos han sido identificados como factores de virulencia en las especies bacterianas. El uso de métodos adecuados como la pirosecuenciación del ARNr 16s permiten conocer qué bacterias se encuentran activas en los procesos cariosos, siendo importante esto a la hora de establecer un tratamiendo adecuado. No obstante, las medidas de prevención son fundamentales en esta enfermedad.
