Utilización de antiagregantes plaquetarios en el Síndrome Coronario Agudo

Abstract

Introducción. El síndrome coronario agudo está considerado una de las principales causas de muerte actualmente. Se han realizado muchos avances farmacológicos en dicha patología, por ello es necesario realizar estudios de adecuación de los nuevos fármacos indicados para el síndrome coronario agudo, con o sin elevación del segmento ST. Objetivos. Comprobar la adecuación de prasugrel y ticagrelor en síndrome coronario agudo. Material y métodos. Estudio observacional retrospectivo donde se incluyeron todos los pacientes mayores de edad que habían sufrido un síndrome coronario agudo e iniciado tratamiento con prasugrel o ticagrelor durante el año 2017. Se estudió la adecuación o no, al tratamiento según las Guías Europeas y el protocolo del Área de Gestión Sanitaria Sur de Sevilla y sus causas. Resultados. Se incluyeron 227 pacientes, 116 con síndrome coronario agudo sin elevación del segmento ST y 111 con elevación del segmento ST. De los primeros, 100 (86%) fueron tratados con ticagrelor y el resto con prasugrel, la no adecuación del tratamiento en síndrome coronario agudo sin elevación del segmento ST con ticagrelor fue del 83% (83 pacientes); la no adecuación con prasugel fue del 17% (17 pacientes); la gran mayoría de ambas a causa de un valor en la escala GRACE<109. Del segundo grupo de pacientes, 98 (88,29%) fueron tratados con ticagrelor y el resto con prasugrel; la no adecuación de ticagrelor y prasugrel respectivamente fue de 51% (50 pacientes) y 38,46% (5 pacientes), ambas en su mayoría por un valor GRACE<109. Conclusión. La mayoría de pacientes se pueden considerar adecuados al no aparecer en la Guía Europea ningún dato sobre la escala GRACE aunque usen un antiagregante más potente que el recomendado en el protocolo del hospital. Un pequeño número de inadecuaciones (3,5%) podían originar problemas de seguridad en el paciente.
