Melatonina y enfermedades neurodegenerativas

Abstract

Las enfermedades neurodegenerativas se caracterizan por la muerte progresiva y selectiva de determinadas poblaciones neuronales. La etiopatogenia de la mayoría de estas patologías es multifactorial y aún desconocida, pero, existen ciertos fenómenos comunes en todas ellas, como son el estrés oxidativo, la neuroinflamación, la agregación proteica y el daño mitocondrial. De entre todos estos procesos patogénicos, el estrés oxidativo desempeña un papel fundamental, por lo que, en los últimos años, se ha estado investigando sobre el posible papel terapéutico de sustancias con capacidades antioxidantes, tales como la melatonina. La melatonina es una indolamina que está involucrada en la regulación de una gran variedad de procesos fisiológicos. Se sintetiza en la glándula pineal y otros órganos y pasa con facilidad a través de cualquier barrera fisiológica, facilitando su llegada a todos los compartimentos y fluidos corporales. Estas características junto con su demostrada inocuidad incluso a altas dosis hacen de la melatonina un candidato perfecto para su uso terapéutico. En esta revisión se recogen numerosas evidencias que demuestran que la melatonina tiene un potente papel como neuroprotector, gracias, no solo a su capacidad antioxidante, sino también a sus acciones antiapoptóticas y antinflamatorias. Sin embargo, la melatonina no es, a día de hoy y a las dosis estudiadas, un tratamiento curativo y su eficacia disminuye en los estadios avanzados de la enfermedad. Así, se requieren futuras investigaciones para seguir avanzando en el tratamiento de las enfermedades neurodegenerativas ya sea aumentando las dosis de melatonina o utilizando la melatonina en combinación con otros fármacos.
