Abstract

En la mamografía la Cicatriz Radial - Lesión Esclerosante Compleja generalmente se presenta como una lesión estrellada o distorsión arquitectural. Por sus características histológicas (lesión planar) y por su forma de presentación radiológica, a veces su identificación es difícil, por lo cual se necesitan radiólogo s expertos acostumbrados a la identificación y manejo de estas imágenes. Debido a estas circunstancias en ocasiones, en el estudio inicial es necesario realizar proyecciones complementarias que certifiquen la existencia de la lesión. Igualmente a veces la identificación resulta difícil en las proyecciones estereotáxicas y en la pieza quirúrgica, la cual en determinados casos ha de ser girada para poner de manifiesto la lesión. No existen criterios radiológicos de fiabilidad que permitan hacer el diagnóstico diferencial entre Cicatriz Radial - Lesión Esclerosante Compleja y lesiones malignas de la mama. Es frecuente la asociación Cicatriz Radial - Lesión Esclerosante Compleja con malignidad o atipia, especialmente con el carcinoma tubular. No podemos establecer claramente si dicha asociación entre Cicatriz Radial - Lesión Esclerosante Compleja y cáncer es debida a la similitud de los hallazgos radiológicos y ecográficos entre ambos procesos, o si bien ante una Cicatriz Radial - Lesión Esclerosante Compleja existe un mayor riesgo de desarrollar un carcinoma de mama. Las Cicatrices Radiales - Lesiones Esclerosantes Complejas por presentarse generalmente como distorsión/estrella, deben ser catalogadas como BIRADS 4 (sospechoso de malignidad). La ecografía, aunque útil para la detección, tampoco aporta datos diferenciales definitivos con el cáncer de mama. Ante datos diagnósticos (radiológicos y/o ecográficos) que sugieran la existencia de una Cicatriz Radial - Lesión Esclerosante Compleja, dada su categoría BIRADS 4, está indicada siempre la realización de estudio histológico.
