Abstract

La Fibrina Rica en Plaquetas y Leucocitos (L-PRF) es un derivado sanguíneo utilizado en múltiples técnicas de cirugía bucal, aunque sus efectos siguen siendo materia de debate en la actualidad. El propósito de este trabajo es evaluar en qué procedimientos de la cirugía bucal este material autólogo aporta beneficios y en qué consisten los mismos. Se realizó una búsqueda de literatura en las bases de batos Pubmed/Medline, Scopus, Cochrane y Web of Science de estudios publicados en los últimos 5 años, obteniendo un resultado de 540 registros, de los cuales 12 se incluyeron en esta revisión. Únicamente se incluyeron ensayos clínicos aleatorizados (ECAs/RCTs). La literatura disponible sugiere que el L-PRF puede mejorar la estabilidad implantaria, sobre todo en las primeras fases de la osteointegración. En la preservación alveolar, el L-PRF puede contribuir a una menor reabsorción ósea postextracción ayudando a mantener las dimensiones alveolares. En la elevación de seno, la adición de L-PRF a otro tipo de injerto óseo no parece producir efectos adicionales, aunque sí produce una maduración más rápida del injerto. En la cirugía de extracción del tercer molar inferior incluido, el L-PRF disminuye la severidad de las secuelas postoperatorias como el dolor o el edema, reduce la incidencia de alveolitis y mejora la regeneración ósea. Sin embargo, se necesitan más ensayos clínicos aleatorizados que confirmen estos resultados.

Leucocyte- and Platelet-Rich Fibrin (L-PRF) is a blood-derived product used in multiple oral surgery techniques, although its effects are still a source of discussion. The purpose of this study is to evaluate in which oral surgery procedures this autologous material provides benefits and what these benefits consist of. A literature search was carried out of studies published in the last 5 years in Pubmed/Medline, Scopus, Cochrane and Web of Science databases. A result of 540 records was obtained, including 12 in this review. Only randomized clinical trials (RCTs) was included. The available literature suggests that L-PRF can improve implant stability especially in the early stages of osseointegration. In alveolar preservation L-PRF reduces post-extraction bone resorption, helping to maintain alveolar dimensions. In sinus lift L-PRF addition to another bone graft does not seem produce additional effects, although it does produce a faster graft maturation. In third molar surgery LPRF reduces the severity of postoperative sequelae such as pain or swelling, decreases the incidence of alveolar osteitis and improves bone regeneration. However, more randomized clinical trials are needed to confirm these results.
