Abstract

La OMS establece que la población de personas mayores de 80 años aumentará 4 veces para el año 2050, lo que desembocará en un aumento de las enfermedades degenerativas relacionadas con el envejecimiento, como la enfermedad de Alzheimer o la Diabetes Mellitus tipo II. Existen evidencias que relacionan ambas patologías. De hecho alrededor del 80% de los enfermos de Alzheimer presentan resistencia a la insulina. Las alteraciones de la vía de la insulina pueden causar un aumento de la formación y acumulación de placas β-amiloides, además de un incremento de la proteína tau hiperfosforilada, siendo estos los principales factores del desarrollo de la enfermedad de Alzheimer. La neuroinflamación, el estrés oxidativo, la senescencia y el desequilibrio de las enzimas acetilcolinesterasa y butirilcolinesterasa son otras vías que relacionan ambas patologías. Esta relación hace que en la actualidad se estudie el uso de fármacos antidiabéticos en el tratamiento de los pacientes con Alzheimer. En esta revisión se han recopilado y analizado estudios actuales sobre la mejora en los indicadores de pacientes de Alzheimer tratados con distintos fármacos antidiabéticos. Entre los fármacos más prometedores encontramos la administración de insulina intranasal, con importantes efectos positivos en la memoria y las sulfonilureas, que han mostrado efectos beneficiosos en el hipocampo. Con respecto a la metformina y las tiazolidinendionas, los últimos estudios exponen que su uso en la enfermedad de Alzheimer es controvertido y que serán necesarios nuevos estudios. Si bien los resultados son prometedores, serán necesarios nuevos estudios con mayor población para poder concluir el beneficio del uso de los fármacos antidiabéticos en la enfermedad de Alzheimer
