Potencial terapéutico de la Capsaicina

Abstract

El protoalcaloide capsaicina es el principal responsable de la pungencia característica de los frutos del género Capsicum. Desde que fue identificada en el siglo XIX, las aplicacio nes de la capsaicina y sus análogos han destacado tanto por su importancia dietética como por su alto potencial terapéutico. La caracterización de sus efectos farmacológicos, lleva ron al descubrimiento de su receptor, el receptor de potencial transitorio V1, también conocido como TRPV1, que se distribuye ampliamente en tejidos del organismo, aunque principalmente en las neuronas aferentes primarias. El uso tópico de capsaicina activa selectivamente a los receptores TRPV1 de las fibras aferentes del dolor, dando lugar a una cascada de eventos que desencadena en la neurona un proceso llamado desfunciona lización. Este proceso ha demostrado tener efectos analgésicos, siendo de utilidad en dis tintas condiciones dolorosas para aliviar el dolor, fundamentalmente, en dolor neuropá tico. El dolor neuropático se manifiesta en una amplia variedad de condiciones clínicas y es un problema importante de salud pública. Actualmente, se recomienda el uso tópico de capsaicina, en cremas, parches, solo, o acompañando a otros fármacos en el tratamiento de ese tipo de dolor. Presenta buen perfil farmacológico, convirtiéndose en una excelente alternativa terapéutica. Además, se han observado efectos beneficiosos en otras patologías como la obesidad, algunos tipos de cáncer y en problemas gastrointestinales, cardiovasculares y dermatoló gicos, entre otros. Muchas de las acciones farmacológicas de la capsaicina son indepen dientes del mecanismo de acción relacionado con el receptor TRPV1. Se requieren ensa yos farmacológicos y clínicos en varias de estas condiciones patológicas para establecer la eficacia de la capsaicina y el mecanismo de acción.
