Abstract

Objetivos: Determinar el conocimiento de los endodoncistas españoles sobre las indicaciones de antibióticos sistémicos y hábitos de prescripción en el tratamiento de las infecciones endodóncicas. Material y métodos: En este estudio transversal descriptivo observacional se solicitó a 100 endodoncistas españoles que respondieran a una encuesta, validada previamente en otras investigaciones publicadas, sobre indicaciones de antibióticos sistémicos en el tratamiento de infecciones endodóncicas. Los datos fueron analizaron mediante el uso de estadísticas descriptivas y la prueba de Chi-cuadrado de Pearson. Resultados: 77 endodoncistas (77%) respondieron la encuesta de forma satisfactoria y fueron incluidos en este estudio. El promedio de duración de la terapia antibiótica prescrita fue de 5,64±1,75 días. En pacientes sin alergias médicas, la mayoría de los encuestados (97.1%) seleccionaron la amoxicilina como el antibiótico de primera elección, solo (61,04%) o asociada con ácido clavulánico (35,06%). El fármaco de primera elección para pacientes con alergia a las penicilinas fue clindamicina 300 mg (74,03%). Para casos de necrosis pulpar con periodontitis apical asintomática, tracto fistuloso y síntomas leves/sin síntomas el 100% de los endodoncistas recetaron antibióticos. Para casos de necrosis pulpar con periodontitis apical sintomática sin absceso 20 endodoncistas prescribieron antibióticos. En el escenario de la prescripción antibiótica profiláctica, el 18,18% de los especialistas prescriben siguiendo pautas no actuales (1 gr 1 hora antes o 1 gr 1 hora antes y 1 gr 1 hora después), en casos no indicados (15,58% en pacientes que toman bifosfonatos orales). Conclusiones: Se ha mejorado notablemente la prescripción de antibióticos por parte de endodoncistas españoles con respecto a estudios anteriores en la última década. Aún así, un bajo porcentaje de profesionales sigue prescribiendo de forma errónea. Es necesaria una mayor concienciación y formación para frenar las resistencias antibióticas.

Aim: To determine the knowledge of Spanish endodontists about the indications of systemic antibiotics and prescription habits in the treatment of endodontic infections. Materials and Methods: In this descriptive observational cross-sectional study, 100 Spanish endodontists were asked to answer a survey, previously validated in other published researches, about indications for systemic antibiotics in the treatment of endodontic infections. Data were analyzed using descriptive statistics and Pearson's Chisquare test. Results: 77 endodontists (77%) answered the survey satisfactorily and were included in this study. The average duration of prescribed antibiotic therapy was 5.64±1.75 days. In patients without medical allergies, most respondents (97.1%) selected amoxicillin as the antibiotic of first choice, either alone (61.04%) or associated with clavulanic acid (35.06%). The first choice drug for patients with penicillin allergy was clindamycin 300 mg (74.03%). For cases of pulp necrosis with asymptomatic apical periodontitis, fistulous tract and mild/symptomatic symptoms 100% of endodontists prescribed antibiotics. For cases of pulp necrosis with symptomatic periodontitis apicalis without abscess 20 endodontists prescribed antibiotics In the scenario of prophylactic antibiotic prescription, 18.18% of specialists prescribe following non-current guidelines (1 gr 1 hour before or 1 gr 1 hour before and 1 gr 1 hour after), in non-indicated cases (15.58% in patients taking oral bisphosphonates) Conclusions: The prescription of antibiotics by Spanish endodontists has been significantly improved over previous studies in the last decade. Even so, a low percentage of professionals continue to prescribe incorrectly. Greater awareness and training are needed to curb antibiotic resistance.
