Abstract

Introducción: la periodontitis y la periimplantitis, son ambas, enfermedades crónicas inflamatorias multifactoriales. Por un lado, la periodontitis es la inflamación del periodonto de soporte del diente incluyendo pérdida de hueso. Por otro lado, denominamos periimplantitis a la inflamación de los tejidos blandos que rodean al implante y en la cual, también se produce pérdida del hueso de sostén. Objetivo: el objetivo principal de este trabajo es valorar si existe un mayor riesgo de que se produzca periimplantitis en un paciente periodontalmente comprometido. Metodología: Esta revisión se ha realizado obteniendo información de la base de datos Pubmed. Para ello, se efectuó una búsqueda minuciosa utilizando una serie de palabras clave así como unos criterios de inclusión y exclusión, la cual dio lugar a la selección de 11 artículos. Resultados: los artículos seleccionados incluyen revisiones sistemáticas, metaanálisis y un estudio comparativo, en los cuales se ha estudiado la posible relación entre presentar un enfermedad periodontal previa y el riesgo de desarrollar periimplantitis, confirmándose la relación entre ambas. Discusión /conclusión: existe mayor riesgo de periimplantitis así como de pérdida de hueso marginal y menor supervivencia del implante en pacientes con periodontitis previa que en pacientes periodontalmente sanos. Esta probabilidad será aún mayor en pacientes con periodontitis agresiva. Además los pacientes fumadores periodontales, también tienen un mayor riesgo de periimplantitis y ésta se produce más frecuentemente si no se sigue un mantenimiento periodontal; Sin embargo, los resultados hay que interpretarlos con cautela debido a posibles limitaciones y factores de confusión.

Introduction: periodontitis and peri-implantitis are both multifactorial chronic inflammatory diseases. On the one hand, periodontitis consists in the inflammation of the supporting periodontium of the tooth including bone loss. On the other hand, the inflammation of the soft tissues surrounding the implant and in which loss of the supporting bone also occurs is called periimplantitis. Objective: the main objective of this work is to assess whether there is an increased risk of peri-implantitis occurring in a periodontally compromised patient. Methodology: This review has been carried out obtaining information from the Pubmed database. For this purpose, a thorough search was performed using a series of keywords and inclusion as well as exclusion criteria, which led to the selection of 11 articles. Results: the selected articles include systematic reviews, meta-analyzes and a comparative study, in which it has been studied a possible relationship between presenting a previous periodontal disease and the risk of developing peri-implantitis, being confirme the relationship between the two. Discussion / conclusion: there is higher risk of peri-implantitis as well as marginal bone loss and shorter implant survival in patients with prior periodontitis than in periodontally healthy patients. This probability will be higher in patients with aggressive periodontitis. Furthemore, periodontal smoking patients also have higher risk of peri-implantitis and this occurs more frequently if periodontal maintenance is not followed. However the results must be interpreted with caution due to possible limitations and confounding factors.
