Abstract

Se ha realizado una revision de la casuistica oncologica maxilo-facial, intervenida por el autor en 5 años, destacando la problematica que para el anestesiologo presenta en particular la cirugia radical del cuello, e interesandose sobre todo por la posibilidad de supervivencia que presentan los pacientes tributarios de cirugia radical bilateral. Para observar la repercusion intracraneal que esta cirugia puede comportar, se ha realizado una investigacion en dos fase: - fase clinica, donde se ha efectuado un seguimiento isotopico de la circulacion cerebral mediante angioganmagrafias dinamicas cerebrales desde el tiempo preoperatorio hasta un postoperatorio tardio, en 9 pacientes intervenidos de d.R.C. (diseccion cervical radical) y en 2 pacientes tras la cirugia radical bilateral. - fase experimental, donde se ha estudiado la evolucion de la presion intracraneal (p.I.C.) en 15 animales de laboratorios (perros comunes) tras la ligadura de una o dos yugulares, efectuadas en el mismo tiempo quirurgico, o en tiempos distintos, demorados entre si diversos periodos de tiempo. Se concluye que estos enfermos, considerados como entidades craneofaciales de alto riesgo, pueden ser intervenidos con las ligaduras venosas yugulares que necesiten, sin que exista repercusion intracraneal, ya que al deber necesitar que sean bilaterales, las ligaduras deben demorarse en el tiempo un periodo minimo de 60 dias.
