Inmunoterapia en el asma bronquial

Abstract

La inmunoterapia surgio en 1911 con vocacion para el tratamiento de las afecciones secundarias al estado atopico. Hoy dia permanece abierta la polemica, en algunos casos visceral, sobre la efectividad de dicha modalidad terapeutica, principalmente en el asma bronquial. Se ha llevado a cabo un estudio prospectivo de casos y controles en el que se incluyeron enfermos de asma sensibles a polenes y acaros de forma independiente, para estudiar en ellos la efectividad de la inmunoterapia con alergenos. Los enfermos fueron controlados a traves de un "score" clinico/terapeutico, de la medicion de los tests cutaneos, niveles de ige total y especifica y del test de provocacion bronquial con metacolina, al inicio y al final de 19 meses de media de seguimiento. Hemos podido comprobar como los enfermos sensibles a polenes fueron capaces de mejorar su clinica e hiperactividad bronquial inespecifica, asi como su sensibilidad cutaneas. Sin embargo, en los enfermos sensibles a acaros la hiposensibilizacion no aporto beneficio alguno a la terapeutica convencional, y solo mostraron una reduccion de la sensibilidad cutanea al alergeno. La inmunoterapia parece mostrar mas efectividad cuando se administra de forma preventiva o sea, antes de la exposicion al antigeno. La ige nos ha aportado muy poca informacion, ya que no mostro variaciones significativas de sus niveles, en ningun grupo de tratamiento. Finalmente decir que los tests cutaneos unicamente se asociaron a la inmunoterapia pero no al beneficio clinico.
