Influencia del VPH en el cáncer oral

Abstract

Introducción: El carcinoma de células escamosas de cabeza y cuello (HNSCC) es el sexto cáncer humano más común en los países desarrollados y es una causa importante de morbilidad en el mundo. Aunque este concepto se usa para explicar tumores en diferentes lugares, es preciso centrarnos concretamente en aquellos localizados en la cavidad oral y en la zona orofaríngea. Estos carcinomas presentan una etiología multifactorial, estando fuertemente asociados con el consumo de tabaco y alcohol. Sin embargo, en los últimos años se han relacionado etiológicamente con la infección por el virus del papiloma humano, afectando sobre todo a la población más joven. Objetivo: analizar la influencia de la infección del virus del papiloma humano (VPH) en la carcinogénesis oral y orofaríngea. Material y métodos: Se ha llevado a cabo una revisión bibliográfica con la ayuda de las bases de datos Pubmed y Scopus para lo cual se determinaron criterios de inclusión y exclusión. Resultados: Del total de 3098 artículos inicialmente obtenidos, solo 15 cumplieron los criterios de inclusión y fueron incluidos en la revisión bibliográfica. Conclusiones: Aunque la mayor parte de la evidencia científica obtenida encuentra cierta influencia en el carcinoma de células escamosas de la cavidad oral y orofaríngea, parece jugar un papel mucho más importante en el cáncer orofaríngeo. Los carcinomas de células escamosas de la cavidad oral y orofaríngea asociados al VPH presentan una entidad tumoral distinta, así como diferentes patrones de diseminación y tasas de supervivencia.

Introduction: Head and Neck Squamous Cell Carcinoma (HNSCC) is the sixth most common human cancer in developed countries and is a major cause of morbidity worldwide. Although this concept is used to explain tumors in different locations, we will focus specifically on those located in the oral cavity and oropharyngeal area. These carcinomas present a multifactorial etiology, being strongly associated to tobacco and alcohol consumption. However, in recent years they have been etiologically linked to an infection caused by the Human Papillomavirus, affecting mainly the youngest population. Objective: to analyze the influence of Human Papillomavirus (HPV) infection on oral and oropharyngeal carcinogenesis. Material and methods: a bibliographical review has been carried out with the help of the Pubmed and Scopus databases for which inclusion and exclusion criteria were determined. Results: of the total of 3098 articles initially obtained, only 15 met the inclusion criteria and were included in the bibliographical review. Conclusions: although most of the scientific evidence obtained finds some influence on both, it seems to play a much more important role in oropharyngeal cancer. HPV-associated carcinomas have a distinct tumor entity, as well as different patterns of spread and survival rates.
