Abstract

OBJETIVOS 1,- Evaluar el rendimiento de la PAT en el diagnóstico microbiológico del AP y del posible carcinoma subyacente. 2,- Analizar y comparar las características clínico-radiológicas y la participación bacteriana de los AP primarios y aquellos asociados a neoplasia bronquial. 3,- Establecer las posibles indicaciones de la PAPT. MATERIAL Y MÉTODOS Serie consecutiva de AP no seleccionadas. En todos se realizó PAPT bajo control radiológico (Fluoroscópico mayoritariamente) con aguja 20-22 G. RESULTADOS Se incluyen 131 pacientes, con edad media de 49.87 años (rango 16-79 años), con factores favorecedores de aspiración orofarínges, 24 con neoplasia pulmonar asociada (no conocida en la presentación del AP en 23 casos), 25 ADVP, 4 con otros factores predisponentes para el AP y 28 sin factor detectable. Los AP adquiridos en la comunidad fueron 115 y en el hospital 16. En 15 casos existía infección HIV asociada. El 32.1% (42/131) presentaba fetidez en el esputo.Las características radiológicas fueron de escasa ayuda para diferenciar los AP asociados a neoplasia anaerobias fue del 58% (65/112) y de aerobias del 65.1% (73/112). Dicha participación varió en función de la etiopatogenia del AP, lugar de adquisición y de la fetidez en el espunto.En los AP adquiridos en la comunidad, las bacterias anaerobias fueron más frecuente de forma significativa en los pacientes con fetidez en el esputo (36/37 vs 22/60, p<0,001) y con circunstancias de aspiración orofaríngea (30/35 vs 28/62, p<0,001) y los bacilos aerobios Gram negativos y/o Staphilococcus aureus se aislaron en una proporción singnificativamente mayor en los pacientes sin fetidez en el esputo (38/60 vs 8/37, p<0,001), sin circunstancias de aspiración (35/62 vs 11/35, p=0,03) y con neoplasia bronquial asociada (15/21 vs 31/76, p=0,02).
