Fisiología del endotelio vascular

Abstract

El endotelio es una monocapa de células situada en los vasos sanguíneos que separa la sangre de otros tejidos y cuya función principal es la regulación y la perfusión sanguínea por todo el organismo a través de cambios del tono vascular, pero también juega un papel fundamental en la homeostasis y en la inflamación. El tejido consta de una gran variedad de células endoteliales con características diferentes, es por ello que se considera un tejido heterogéneo, lo que le permite ejercer funciones diferentes en el organismo. Durante la disfunción endotelial se produce una pérdida del balance entre vasodilatación y vasoconstricción, de manera que el estado vasoconstrictor pasa a ser dominante, conllevando cambios fisiopatológicos progresivos que expresan estados de proinflamación, prooxidación, procoagulación y adhesión celular. Uno de los factores de riesgo más importantes en el desencadenamiento de la disfunción endotelial es el estrés oxidativo, clave en el desarrollo de múltiples enfermedades cardiovasculares como obesidad, hipertensión o aterosclerosis. El aumento en la producción de especies reactivas de oxígeno (ROS) favorece la liberación de factores inflamatorios y disminuye la biodisponibilidad de óxido nítrico (NO), factor vasodilatador esencial en la regulación vascular. De este modo, mantener la funcionalidad del endotelio es esencial, pudiendo contribuir a evitar el desarrollo de enfermedades cardiovasculares. La función endotelial puede ser medida mediante su capacidad para ejecutar varias funciones como la expresión de moléculas de adhesión, el mantenimiento de un ambiente antitrombótico o la regulación del tono vascular, para la cual se hace una valoración de la vasodilatación dependiente de endotelio usando distintos agonistas de la liberación de NO.
