Abstract

The purpose of this study was to examine the aspirated state of the glass particles poured into drug ampoule with the drug is injected into the injector when the drug ampoule is broken, the size and shape of glass particles, whether it removes glass particles by use of filter. Six randomly selected drug ampoules that the direction of opening is indicated by a dot were used in the study. The injectors were divided into two groups, one with a filter and one with a needle. Each drug was aspirated from ampoule into the syringes using a 22 gauge needle. Each of them was transferred to the a sterile cell culture plate, examined under a invert light microscope, and their pictures was taken. It was observed that all of the drug ampoules were properly opened by breaking at neck portion, a visible glass particle did not occur during the fracturing, there was no accumulation of any glass particles in the drug ampoules. In the microscopic field scanning, It was detected 3 splinter-form glass particles that were 563.03 ± 356.79 ?m in one of nine areas of the group with filter. It was detected 18 glass praticles that were 35.20 ± 10.54 ?m in different shapes and sizes on the whole of nine areas of group with needle. We could not accessed any study that reporting the presence of splinter-form glass particle in the literature search. It has proven by this study what drug with glass particles and splinter-form particles also can be aspirated into syringe from glass drug ampoule, and even if the filter is used these glass particles be transferable risk for patients. A new drug ampoule form and filter systems need to be developed for the prevention of transfering of glass particles to the patient in the practice of parenteral drug therapy.

Bu çalışma, ilaç ampulünün kırılarak açılması esnasında içine dökülmüş mikro boyutta cam parçacıkların ilaçla birlikte enjektöre çekilme durumunu, cam parçacıklarının boyutlarını ve şekillerini, filtre kullanımının cam parçacıklarını bertaraf edip etmediğini incelemek amacıyla yapılmıştır. Çalışmada açılma yönü nokta işaretli rasgele seçilmiş altı adet ilaç ampulü kullanıldı. Enjektörler filtreli ve iğne uçlu olmak üzere iki gruba ayrıldı. Her bir ampul içeriği 22 gauge iğne ucu kullanılarak enjektörlere çekildi. Her biri steril hücre kültürü pleytine aktarıldı ve invert ışık mikroskobu altında incelenerek resimleri çekildi. Ampullerin tamamının boyun kısmından düzgün biçimde kırılarak açıldığı, kırılma esnasında gözle görülür bir cam parçacığı oluşmadığı, ampul içinde herhangi bir cam parçacık birikimi olmadığı gözlemlendi. Mikroskobik alan taramasında filtreli gruba ait dokuz alanın birinde kıymık şeklinde 563,03±356,79 µm boyutunda 3 adet cam parçacık tespit edildi. İğne uçlu gruba ait dokuz alanın tümünde farklı şekil ve büyüklüklerde 35,20±10,54 µm 18 adet cam parçacık tespit edildi. Literatür taramasında kıymık şeklinde cam parçacıkların varlığını gösteren herhangi bir çalışmaya ulaşılamamıştır. Cam ilaç ampullerinden ilaçla birlikte cam parçacıkları ve cam kıymıklarının da enjektöre çekilebildiği ve bu cam parçacıklarının filtre kullanılsa da hastalara aktarılabileceği riskinin mevcut olduğu kanıtlanmıştır. Parenteral ilaç tedavisi uygulamasında cam parçacıklarının hastaya aktarımını önlemek için yeni bir ampul formu ve filtre sistemlerinin geliştirilmesine ihtiyaç vardır.
