El objetivo del presente trabajo es profundizar el análisis del modelo de "gobernanza en salud", y su incidencia en la equidad en salud pública en América Latina. Mi hipótesis general es la siguiente: si queremos avanzar en la equidad en salud pública para nuestra región, entonces debemos abandonar el extrapolado uso de "gobernanza en salud" como herramienta estratégica o modelo paradigmático: dado sus orígenes, connotaciones ideológicas, limitaciones de aplicación. Pues la "gobernanza en salud" encubre, en su raíz histórica, una crítica al Estado que conlleva la limitación de sus poderes; en especial, sus poderes distributivos y su responsabilidad social- otorgando la exclusiva responsabilidad de la salud en el individuo (estilos de vida). En su lugar, proponemos justificar normativamente un "modelo sanitario, equitativo y participativo para América Latina", que responda a nuestras necesidades e intereses, y defienda la responsabilidad social en salud pública (determinantes sico sociales).
The aim of the present work is to deepen the analysis of the model of "governance in health", and his impact on equality in public health in Latin America. My general hypothesis is the following one: if we want to advance in the model of equality in public health for our region, then we must leave extrapolated use of "governance in health" as strategic tool or paradigmatic model (in view of his origins, ideological connotations, limitations of application). The "governance in health" it conceals, in his historical root, a critique to the State that carries the limitation of his power (especially, his distributive power and his social responsibility), granting the exclusive responsibility of the health in the individual (ways of life). In his place, we propose to justify a normative model of equality in health care for Latin America, that encourage community participation according to our needs and interests, and defend the social responsibility in public health (social determinants).
