Abstract

La gestión hegemónica piensa que en las organizaciones sanitarias, el trabajo de concepción lo realiza el personal médico, mientras que el trabajo de enfermería es de pura ejecución. Esto configura un modelo que divide a los grupos ocupacionales de acuerdo al grado de control y autonomía sobre sus procesos de trabajo. El objetivo del artículo es analizar las relaciones de supervisión y confianza entre las médicas residentes y las enfermeras que se desempeñan en la terapia neonatal de un hospital del conurbano bonaerense; enfocándonos en la supervisión que realizan las primeras a las segundas. El abordaje es cualitativo, mediante la realización de entrevistas semiestructuradas a ambos grupos ocupacionales. Entre los resultados más sobresalientes pudimos dar cuenta de la autonomía responsable de las médicas residentes, que las insta a ser trabajadoras con un compromiso proactivo hacia la atención del paciente, y por ende a responsabilizarse de supervisar a enfermería. Las médicas residentes no controlan por igual a cualquier enfermera, las dos distinciones que operan son la experiencia y el interés por el paciente. Cuándo las médicas reconocen el aporte distintivo que realiza enfermería a la producción del cuidado, es posible la construcción de una confianza colectiva.

Hegemonic management thinks that in healthcare organizations, the work of conception is performed by medical personnel, while nursing work is purely execution. This configures a model that divides occupational groups according to the degree of control and autonomy over their work processes. The aim of this article is to analyze the supervision and trust relationships between resident physicians and nurses working in neonatal therapy in a hospital in the Buenos Aires metropolitan area, focusing on the supervision of the former by the latter. The approach is qualitative, through semi-structured interviews with both occupational groups. Among the most salient results, we were able to report the responsi- ble autonomy of the medical residents, which urges them to be workers with a proactive commitment to patient care, and therefore to take responsibility for supervising nursing. The two distinctions that operate are experience and interest in the patient. When physi- cians recognize the distinctive contribution nursing makes to the production of care, it is possible to build collective trust.
