Hemocromatosis Hereditaria en la Población Adulta de Puerto Madryn

Abstract

Hereditary hemochromatosis (HH) is an autosomal recessive disorder that generates excessive absorption of iron and its accumulation in various vital organs. The effects attributed to this pathology include the increased risk of liver cancer, arthritis, cardiomyopathy, diabetes and other consequences that decrease life expectancy in ten years. However, its early detection allows specific treatment of the disease. Several studies conducted in Europe have shown that populations of Celtic origin have the highest frequency (10-13%) of the causal genotypes of HH (allele C282Y and H63D). However, the incidence of risk markers for the disease in Native American and admixed populations has been little explored. The population of Puerto Madryn is of special interest, because it presents a European foundational origin, mainly Welsh, Spanish and Italian and it presents a high Native American component. In the present study, 75 blood samples were analysed from unrelated voluntary donors who attended the donation campaigns for bone marrow of the Zonal Hospital. By studying of 46 markers of ancestry, individual genetic miscegenation (European, Native American and African) was estimated; and by analysing the regions of the short arm of chromosome 6 (6p21.3), the presence of risk alleles for HH was determined. The level of iron in blood was analysed by transferrin quantification. In addition, each participant was interviewed about his lifestyle and genealogy. The results obtained showed the presence of risk markers for HH in 40% of the samples from Puerto Madryn, with a higher frequency of the H63D allele (36%) followed by C282Y (6%). No correlation was observed between the presence of risk alleles, lifestyle and family history. A marked influence of European ancestry (85%) was noted in the carriers of the risk polymorphisms. This work constitutes a first approach to the study of HH in a population of Patagonia, addressing the individual and population ancestral component. The results highlight the need to know the human populations structure to understand the incidence of hereditary diseases both local and migrant, especially in countries such as Argentina with multiethnic and multicultural biological constitution.
