La glicproteina-P: implicancia en la farmacocinetica y farmacodinamia de drogas

Abstract

La glicoproteina-P es el transportador de eflujo perteneciente al grupo de proteinas ligadoras de ATP mas estudiado. La proteina funiciona como transportador de eflujo disminuyendo las concentraciones intracelulares de los sustratos. La localizacion anatomica y su actividad de extrusion de toxinas y xenobioticos del interior celular, sugieren que la proteina podria estar involucrada en la proteccion del organismo contra los xenobioticos por limitar su entrada y contribuir a su excresion. Ademas, dada la amplia distribucion y expresion en tejidos normales, desempeña un papel importante en la disposicion y la eliminacion de drigas. La expresion aumentada de la glicoproteina-P en tejidos tumorales, en muchos casos, se relaciona con mecanismos de resistencia a los farmacos. Ciertas interacciones entre drogas se atribuyen a la inhibicion e induccion de la glicoproteina-P. Para evaluar la incidencia de la modulacion del transportador en la terapeutica farmacogica de la enfermedad se estan desarrollando diversos estudios clinicos. El objetivo de este articulo es realizar una reseÑa sobre el estado de conocimiento de la glicoproteina-P y de los compuestos sustratos, inhibidores e inductores, su papel en los mecanismos de absorcion, distribucion, metabolizacion y eliminacion de farmacos y las interacciones entre drogas donde se ha detectado su participacion.
