Helsesykepleieres erfaringer med psykiske helseplager blant ungdom med innvandrerbakgrunn på ungdomsskolen

Abstract

Abstract Background: Public health nurses in Norway today encounter a school environment abundant in diversity. Individuals with an immigrant background show higher rate of mental health problems through their lives when compared to the majority population in Norway. The literature search has shown that's there a lack of research that specifically addresses how public health nurses identify and manage mental health issues among adolescents with an immigrant background in secondary school. Purpose: This research aimed to explore public health nurses’ experiences in confronting and identifying mental health issues among secondary school adolescents with an immigrant background. The aim of said research is to contribute a greater understanding of the experiences that nurses have related to this topic, while also increasing awareness of mental health among adolescents with an immigrant background. Method: The study is based on a qualitative research method with a phenomenological perspective. Data was collected using structured individual interviews with seven public health nurses. The data from the interviews was then analyzed in accordance with Braun and Clarke’s reflexive thematic analysis. Results: The analysis resulted in four main themes, 1) Public health nurses’ approach to adolescents with an immigrant background, 2) From curiosity to trust and relationship, 3) Public health nurses’ perception of their own competence and 4) Public health nurses’ collaboration partners. The study revealed that public health nurses identified a need for adaptations in conversations adolescents with an immigrant background, considering language barriers, word formulations and relationship building. The term mental health was rarely used due to its association with stigma. Interpreting conversations was necessary, but resource intensive. The public health nurses agreed that they lacked assessment tools and guidelines for addressing mental health among adolescents with an immigrant background. A desire and need for increased intercultural competence among public health nurses emerged. The importance of strong collaborative partners, such as teachers and parents around the youth, was highlighted as valuable, even though collaboration with parents appeared to be challenging. Conclusion: The study indicates a need for increased attention to the topic and concept of mental health among immigrant youth. Public health nurses express the need for more competence, tools, and guidelines tailored to a multicultural environment of adolescents in secondary schools. Sammendrag Bakgrunn: Helsesykepleierne i Norge møter i dag et skolemiljø som kjennetegnes av mangfold. Personer med innvandrerbakgrunn har med økende alder høyere forekomst av psykiske helseplager sammenlignet med majoritetsbefolkningen i Norge. Litteratursøk har vist at det er mangel på forskning som spesifikt adresserer hvordan helsesykepleiere identifiserer og håndterer psykiske helseplager blant innvandrerungdom i ungdomsskolen. Hensikt: Målet med studien var å utforske helsesykepleiernes erfaringer med å møte og identifisere psykiske helseplager hos ungdommer med innvandrerbakgrunn i ungdomsskolen. Herunder bidra til økt innsikt i hva helsesykepleiere opplever knyttet til dette temaet, samt øke helsesykepleiernes bevissthet om psykisk helse blant innvandrerungdom. Metode: Studien har et kvalitativ forskningsdesign med et fenomenologisk perspektiv. Data ble samlet inn gjennom semi-strukturerte individuelle intervju med syv helsesykepleiere. Analysen av datasettet fra intervjuene ble gjennomført i henhold til Braun og Clarke sin refleksive tematiske analyse. Resultater: Analysen resulterte i fire hovedtema, 1) Helsesykepleiers tilnærming til ungdom med innvandrerbakgrunn, 2) Fra nysgjerrighet til tillit og relasjon, 3) Helsesykepleiernes oppfatning av egen kompetanse og 4) Helsesykepleierens samarbeidspartnere. Studien avdekket at helsesykepleierne identifiserte behov for tilpasninger i samtale med ungdom med innvandrerbakgrunn med tanke på språkbarrierer, ordformuleringer og relasjonsbygging. Begrepet psykisk helse ble sjeldent brukt som følge av stigma. Tolkesamtaler var nødvendige, men ressurskrevende. Helsesykepleierne var enige om at de manglet kartleggingsverktøy og egne retningslinjer for psykisk helse til ungdom med innvandrerbakgrunn. Det kom fram et ønske og behov for økt interkulturell kompetanse blant helsesykepleiere. Betydningen av gode samarbeidspartnere som lærere og foreldre rundt ungdommen var verdifullt, dette til tross for at samarbeid med foreldrene fremstod som utfordrende. Konklusjon: Studien indikerer at det er behov for økt oppmerksomhet rundt temaet og begrepet psykisk helse blant innvandrerungdom. Helsesykepleiere ytrer behov for mer kompetanse, verktøy og retningslinjer som er tilpasset et flerkulturelt ungdoms- og foreldremiljø i ungdomsskolen.
