Background: Decision- making processes to end life-prolonging treatment in intensive care units involves demanding ethical and emotional assessments. Advanced medical technology enables extended life but can also lead to overtreatment and challenges related to a dignified end of life. In such situations the collaboration between doctors and intensive care nurses is essential for good and well thought out decisions. Good interaction is important to balance medical judgments with the best interest for the patients. An aging population and increased disease survival places further pressure on intensive care services, which reinforces the need for structured decision -making processes and effective interprofessional collaboration. Purpose: The study examines intensive care nurses and anesthesiologist`s experiences with collaboration in the process of determining to end life- sustaining treatment in critically ill patients in the intensive care unit. Method: A qualitative method with a descriptive design was used in this study. Data was collected through structured individual interviews. A total of 11 participants were recruited, whereas 7 nurses and 4 anesthetists participated from three different intensive care units at hospitals in the South-East of Norway. The interview sessions were recorded with the use of two external recorders and were transcribed continuously. The analyses were carried out using Braun & Clark`s (2022) guide to reflexive thematic analysis with an inductive approach. Results: Four themes were identified; Experience strengthened cooperation through open communication and mutual inclusion, Taking time to reach an agreement promotes a good decision-making basis in the collaboration , Difficult decisions can lead to prolonged treatment and frustration in collaboration, and National guidance and supporting tools can help to strengthen collaboration. Conclusion: Mutual inclusion, openness and communicating in meeting each other resulted in good doctor-nurse collaboration in the decision-making process. It was important for the doctor- nurse collaboration that the decision to end life-prolonging treatment was made on a good interdisciplinary basis and that the time required for everyone to come to an agreement was used. At the same time, the decisions are emotionally and ethically demanding, and prolongation of treatment created frustrations in the collaboration, especially among the intensive care nurses. There is a need to make the National guidance better known in the clinical environment and a more structured approach can strengthen collaboration in the decision- making process.
