Abstract

Bakgrunn: Samhandlingen mellom sykehus og kommunehelsetjenesten er en kontinuerlig og avgjørende prosess i helsetjenesten. Sykepleiere har en lovpålagt og sentral rolle i å koordinere og planlegge behandlingen på tvers av tjenestenivåene, for å sikre en trygg og helhetlig pasientbehandling. Likevel finnes det betydelige utfordringer i dagens samhandling, særlig knyttet til kommunikasjonen mellom sykepleiere i ulike deler av helsetjenesten. For at sykepleiere skal kunne bidra til forbedring, er det nødvendig med kunnskap om de aktuelle utfordringene. Med økt forståelse kan man arbeide målrettet for å forbedre kommunikasjonen og styrke samarbeidet mellom tjenestenivåene. Hensikt: Undersøke hvilke sentrale utfordringer sykepleiere møter i kommunikasjonen ved samhandling mellom sykehus og kommunehelsetjenesten. Metode: Litteraturstudium basert på ni forskningsartikler funnet i perioden mars 2025 til mai 2025. Resultat: Resultatene viste at utfordringene i kommunikasjonen ved samhandling hovedsakelig knyttes til tre områder: 1) kommunikasjonsverktøyene strekker ikke til, 2) manglende forståelse knyttet til kommunikasjonsinnhold, 3) arbeidsvilkår svekker kommunikasjonen. Konklusjon: Litteraturstudiet viser at sykepleiere møter betydelige kommunikasjonsutfordringer i samhandlingen mellom sykehus og kommunehelsetjenesten, knyttet til teknologiske barrierer, mangelfullt informasjonsinnhold og krevende arbeidsforhold. Den mest sentrale utfordringen er knyttet til kvaliteten av informasjonsinnholdet, som ifølge studiene kan true pasientsikkerheten og svekke kontinuiteten i pasientforløpet. Background: Collaboration between hospitals and municipal healthcare services is crucial and occurs continuously. Nurses play a legally mandated and central role in coordinating care across service levels to ensure safe and holistic patient treatment. Despite this, communication between nurses in different parts of the healthcare system remains a major challenge. To improve this collaboration, greater insight into these communication barriers is essential. Aim: To examine the central challenges nurses face in communication during collaboration between hospitals and the municipal healthcare service. Method: Literature review based on nine research articles found between March 2025 and May 2025. Results: The results showed that the communication challenges are mainly related to three areas: 1) communication tools are inadequate, 2) lack of understanding related to content, 3) working conditions weaken communication. Conclusion: The literature review shows that nurses face communication challenges in the collaboration between hospitals and municipal healthcare services, particularly related to technological barriers, insufficient information content, and demanding working conditions. The most critical challenge is related to the quality of the information, which can threaten patient safety and weaken continuity in the treatment.
