Prehospital administrasjon av Fascia Iliaca Compartment Blokade

Abstract

Bakgrunn Norge er et av landene med høyest forekomst av hoftebrudd i verden, med ca. 9000 tilfeller i året. Hoftebrudd, også kalt proksimal femurfraktur, er en smertefull tilstand som krever effektiv smertelindring og rask transport til sykehus. Sterke opioider brukes ofte prehospitalt, men kan gi pasienten betydelige bivirkninger. Inhospitalt anbefales nerveblokade, eksempelvis Fascia Iliaca Compartment Blokade (FICB), som smertelindring for å redusere bruk av opioider ved hoftebrudd. Dette reiser spørsmål om hvorvidt nerveblokade kan implementeres prehospitalt til fordel for opioider ved hoftebrudd. Problemstillingen vår er: Kan prehospital Fascia Iliaca Compartment Blokade administrert av paramedisinere være en trygg metode for prehospital smertelindring av pasienter med proksimal femurfraktur? Metode Oppgaven bruker litteraturstudie som metode. PICO-skjema ble brukt for å strukturere litteratursøket som ble gjennomført i databasen PubMed. Totalt fire forskningsartikler ble inkludert etter nøye gjennomgang. Resultat Studiene viser at FICB kan utføres prehospitalt av paramedisinere, sykepleiere og leger med høy suksess. Opplæringen varierer, men prosedyren ser ut til å være effektiv og ha lav risiko for uønskede hendelser, sammenlignet med opioider. FICB gir betydelig smertelindring, med reduksjon i smerteskår i prehospital sammenheng. Konklusjon FICB fremstår som en trygg og effektiv prosedyre, som kan gjennomføres av paramedisinere med høy suksess. Det fremkommer at paramedisinere har godt nok kunnskapsgrunnlag til å håndtere risikoene som er forbundet med FICB. Funnene viser at FICB er trygg, med betydelig smertelindring og lav risiko for uønskede hendelser. Ytterligere forskning i Norge er nødvendig for å kunne avgjøre om FICB er egnet som prehospital smertelindring ved hoftebrudd i den norske ambulansetjenesten.
