Risikovurdering av løfteoperasjon på Åsgard A

Abstract

This bachelor's thesis presents two risk-based analyses of a lifting operation that Oceaneering is assisting Equinor with. The operation takes place on one of Equinors production vessels, called Åsgard A. The analyses main goal is reducing risk of hazards for both personnel and equipment, as well as minimizing the generators down time. The problem is solved with the help of 6 outcome goals. The situation analysis is completed, and the required documetation from the previous lifting operation regarding generator HGA is collected. This documentation formed a basis for the implementation of both the FMECA and the risk assessment. The FMECA results indicates that the mechanism for fastening the loads is the component with the highest risk evaluation on the lifting yoke due to the possibility of rust/corrosion. The risk assessment was done with some elements sourced from HAZOP. The use of guide words from the HAZOP had little to no contribution to the analysis. The unwanted event with the biggest connected risk was due to weather conditions, waves and wind. There was some suggestions of actions to implement before starting the operation. This includes implementing clear and definite limits for waveheight, as well as certification of the second lfiting yoke. Personnel with both practical and theoretical experience has quality assured the analyses. Two analyses is completed, a FMECA which analyses on a component level and a risk assessment analysing the lifting operation as whole. The FMECA is used for identifying failure modes in the lifting equipment. The most significant failure mode was failure in the mechanism for attaching the loads to the lifting yoke. This failure mode was caused by rust/corrosion. The risk assessment was inspired by the HAZOP method, where the group attempted to create an innovative tool for further analyses. According to the analysis, there are two substantial unwanted events. These events are regarding collision with structure/personnel, caused by waves/weather and wind. There was some proposals of actions to implement before starting the operation. This includes implementing clear and definite limits for wave height, as well as certification of the second lfiting yoke. Oceaneering has provided good feedback of the work performed. Some of these actions will be taken to consideration before starting the operation. Personnel with both practical and theoretical experience has quality assured the analyses.
