Atferdsanalytisk behandling av depresjon hos personer med psykisk utviklingshemming

Abstract

Personer med psykisk utviklingshemming har lignende forekomst av psykiske lidelser og depresjon som resten av befolkningen. Likevel, er det kjent at det er mangelfull behandling av depresjon hos personer med psykisk utviklingshemming, spesielt til personene som har særlig nedsatt funksjonsnivå. Mange behandlingsformer for depresjon, inkludert atferdsaktivering, krever aktiv deltagelse fra pasienten i behandlingen, og dette kan være vanskelig å følge for personer med særlig nedsatt funksjonsnivå. Atferdsorientert tilnærminger i behandling av depresjon inneholder prinsipper som med tilpasninger bør ha en effekt også på personer uten adekvate ferdigheter til å gjennomføre en normert behandling av atferdsaktivering. Artikkelen presenterer historikken, rammeverket og hvordan atferdsaktivering ser ut i dag. Det blir også presentert en atferdsanalytisk forståelse av depresjon. Avslutningsvis vil det bli presentert hvordan grunnprinsippene fra atferdsaktivering kan tilrettelegges for en sårbar gruppe med mennesker som mangler adekvate ferdigheter til å ta ansvar for sin egen psykiske helse. A conservative estimate indicates that individuals with intellectual disabilities have equal or somewhat higher prevalence of mental illness and depression then rest of the population. It is also known that treatment for depression for individuals with intellectual disability is insufficient, especially for individuals with significant reduced adaptive skills. Most forms of treatment, including behavioral activation, is dependent on participation in the treatment. The framework of behavioral activation includes principles that with some adaptions could have an effect for people without the adequate skills to take care of their own mental health. The article will also give a brief look at the history, give a presentation of the framework and explain how behavioral activation looks like today. The article will also give a behavioral analytic understand of depression. In conclusion the core principles of behavioral activation will be explained in a form that may be adapted for people lacking the adequate skill to take care of their own mental health.
