Trygt behandlingsforløp for voksne pasienter med ekstern ventrikkeldrenasje i en intensivavdeling -En kunnskapsbasert fagprosedyre-

Abstract

Background: Low admittance of intensive care patients with external ventricular drain and a lack of evidence-based guidelines in our resident intensive care unit, is the background for our topic choice. It is of vital importance that intensive care nurses utilize guidelines, for patient safety and to secure quality of care. Aim: The aim of this study was to produce an evidence-based protocol that can minimize variations in practice and ensure appropriate treatment of intensive care patients with external ventricular drain. Methods: This guideline is developed according to «Metode og minstekrav for utarbeidelse av kunnskapsbaserte fagprosedyrer» grounded in the «Appraisal of Guidelines for Research & Evaluation» II tool. An extensive literature search with the assistance of a specialized librarian and consensus from among our project group, provided fundament of evidence for the protocol. The literature search produced three guidelines, one literature review and one scoping review. Results: An evidence-based protocol, flow diagram and checklists were established for the treatment of intensive care patients with external ventricular drainage. The protocol provides five focus points: Establishing care of patients with external ventricular drain in the intensive care unit, intracranial pressure monitoring with an external ventricular drain, problem-solving related to non-infectious complications, intrahospital transport of patients with external ventricular drain, and weaning and removal of external ventricular drain. Conclusion: Intensive care nurses can utilize this evidence-based protocol to ensure safe treatment of patients with external ventricular drain in the intensive care unit. Application of the protocol will enhance patient safety and increase the quality of care, whilst minimizing variations in practice.
