Abstract

Background: Procedures regarding admission of new patients in an intensive care unit has historically been locally determined. In the light of standardization of clinical assessment of patients in general wards developed by the patient safety program “in safe hands 24/7”, we wanted to look at what characterizes procedures for admission of intensive care patients in an intensive care unit. Purpose: The purpose of this study was to look for characteristics in procedures for admission of intensive care patients in different intensive care units in Norway. Method: This qualitative content analysis compared procedures from intensive care units in different hospitals, to look at variations and similarities in the procedures. 16 intensive care units were asked from 14 different hospitals. 13 intensive care units answered, and 8 procedures from 7 different hospitals were included in this study. Results: Common to these procedures were that they described the following; formal requirements, taking care of family members, allocate recourses, preparation for action and having a defined plan for the patients. The procedures had a particular focus on use of the ABCDE-principals in clinical assessment and describing team work to ensure patient safety. Conclusion: The study concluded, despite the similarities and common focus areas, there is a need for further research on the topic and development of a uniform procedure.

Bakgrunn: Mottakssituasjoner av nye pasienter på intensivavdelingen og prosedyrer rundt dette har historisk sett vært lokalt betinget. I lys av pasientsikkerhetsprogrammet «i trygge hender 24/7» sin standardisering av observasjon på sengepost, ønsket vi å se på hva som kjennetegner prosedyrer for mottak av intensivpasienter på intensivavdeling. Hensikt: Hensikten med studien var å se etter kjennetegn i prosedyrer for mottak av intensivpasienter på ulike intensivavdelinger i Norge. Metode: Denne kvalitative innholdsanalysen sammenlignet prosedyrer fra intensivavdelinger på flere sykehus for å avdekke likheter og variasjoner. 16 intensivavdelinger fordelt på 14 sykehus ble forspurt, 13 intensivavdelinger svarte og 8 prosedyrer fra 7 sykehus ble inkludert i denne studien. Resultat: Felles for disse prosedyrene var at de beskrev; formelle krav, oppfølging av pårørende, ressursfordeling, handlingsberedskap og at de definerte plan for pasienten videre. Det forelå spesielt fokus i prosedyrene rundt bruken av ABCDE-prinsippet i kliniske vurderinger, og det å beskrive samarbeid for å ivareta pasientsikkerheten. Konklusjon: Studien kom frem til at på tross av flere likhetstrekk og sammenfallende fokusområder, foreligger det et behov for videre forskning rundt temaet og utviklingen av en enhetlig prosedyre.
