Hva er intensivsykepleiers erfaring med oppfølging av pårørende og oppfatninger til å innføre etterlattesamtaler?

Abstract

Background: During a patient`s illness period in hospital and after death, care and support should be provided to the patient`s family members as a part of the integral role in intensive care nursing. Healthcare workers must offer bereavement support, especially after traumatic deaths. Family members should be given the opportunity to ask questions and reflect upon the illness period and death. Aim: The aim of this study was to investigate the support given to family members around the time of death of their loved one, and current practice of formal bereavement support. Methods: An exploratory design was employed, and data were obtained with 7 individual, semi-structured interviews. Content analysis by Graneheim and Lundman was performed to identify common themes. Results: The analysis of the individual interviews yielded two themes: Intensive care nurse`s perception and experience of the situation regarding a death of a patient is of importance and will affect the support given to the bereaved family members, and intensive care nurses recommend formal, organized bereavement support to be conducted. The findings found the support received by the family members to be immediate and by chance. In order to implement formal bereavement support, intensive care nurses must overcome several barriers. Bereavement follow-up can assist the family members in working through the grief process. Conclusion: The study showed that there was a perceived difference in supporting family members after an expected death of an ill patient compared to when death was unexpected. The intensive care nurses personal qualities had an impact on how the bereaved family members were supported. No bereavement services are as of yet implemented in the ward, hence the random and varied support given by the intensive care nurses to the bereaved families. If family members wanted to receive bereavement follow -up they must take the initiative, but ideally this contact ought to be initiated by the intensive care nurse/ ward. Bereavement follow-up can assist the family members in working through the grief process.

Bakgrunn: Ivaretagelse av pasientens pårørende når pasienten er innlagt, og etter et eventuelt dødsfall, er en del av intensivsykepleierens ansvarsområde. Helsepersonell bør ivareta og følge opp etterlatte etter dødsfallet blant annet ved å tilby etterlattesamtale, særlig etter dramatiske dødsfall, slik at de etterlatte får stille spørsmål og reflektere rundt sykeleiet og dødsfallet. Hensikt: Studiens hensikt var å belyse hvordan pårørende ble ivaretatt umiddelbart etter et dødsfall og hvilke rutiner som eksisterte for utførelse av planlagte etterlattesamtaler. Metode: Masterstudien har et kvalitativt design. Data ble innhentet med syv semistrukturerte individuelle intervju. Datamaterialet ble analysert ved hjelp av kvalitativ innholdsanalyse av Graneheim og Lundman. Funn: Analysen av de individuelle intervjuene resulterte i to temaer; Intensivsykepleiers opplevelse og tolkning av situasjonen i forbindelse med dødsfall er avgjørende for ivaretagelse av pårørende og intensivsykepleierne anbefaler at oppfølging av pårørende bør være planlagte etterlattesamtaler. Masterstudien fant at det ble gitt tilfeldig og umiddelbar støtte til de pårørende etter et dødsfall. Informantene identifiserte ulike barrierer som må overvinnes for at planlagte etterlattesamtaler skal kunne innføres. Intensivsykepleierne kan bidra til videre støtte av pårørende i sorgprosessen ved å tilby etterlattesamtaler Konklusjon: Masterstudien fant at det for intensivsykepleierne var forskjell på å ivareta pårørende ved et forventet dødsfall i forhold til dødsfall som kom uventet. Intensivsykepleiernes personlige egenskaper påvirket hvordan pårørende ble ivaretatt. Mangel på rutine gjorde at støtten ble tilfeldig og varierende. Ved behov for videre oppfølging fra avdelingen var det overlatt til de etterlatte å ta kontakt, denne kontakten burde vært initiert av avdelingen. Etterlattsamtaler kan hjelpe pårørende i sorgprosessen.
