Abstract

Bakgrunn: Gjennom flere praksisperioder har vi erfart utfordringer rundt utøvelsen av den ideelle sykepleie. Dette grunnet manglende tid da det legges stor vekt på effektivisering. Sykepleie av kvalitet kjennetegnes blant annet ved ivaretakelse av pasientsikkerhet, pasientinvolvering- og innflytelse. Kvalitet forutsetter også at sykepleier har tilstrekkelig tid. Hensikt: Hensikten med denne studien er å belyse hvordan tidspress påvirker kvalitet og pasientsikkerhet i sykepleien. Metode: En systematisk litteraturstudie. Studien inkluderer åtte forskningsartikler. Resultat: Flere studier viser en sammenheng mellom sykepleiernes arbeidskrav og tidspress. Andre studier peker også på sykepleiernes nedsatte evne til sykepleier-pasient-interaksjoner som følge av manglende tid. Profesjonsforventninger og et påfølgende tidspress kan påvirke sykepleiernes oppfatning rundt egen utøvelse. Tidspresset kan også medføre negative strategier i arbeidet. Konklusjon: Sykepleiere kan oppleve et tidspress som påvirker kvalitet og pasientsikkerhet. Særlig gjennom manglende tid til sykepleier-pasient-interaksjoner, arbeidshelse, og negative strategier med konsekvenser for individualisering og muligens sykepleiernes observasjon- og vurderingsevner. Dokumentasjon- og papirarbeid blir beskrevet som å være belastende på sykepleiernes tid, men er likevel essensielt for kvalitet og pasientsikkerhet. Et støttende arbeidsmiljø er viktig for sykepleiernes håndtering av tidspresset. Nøkkelord: Tidspress, sykepleier, kvalitet, pasientsikkerhet, arbeidshelse.

Background: Throughout several periods of practical training we have experienced a difficulty of practicing ideal nursing. This is due to the lack of time as a result of an emphasis on efficiency. Quality nursing is characterised, among other things, by safeguarding patient safety, patient involvement- and influence. Quality also assumes that the nurse has sufficient time. Aim: The aim of this study is to demonstrate how time pressure affects quality and patient safety in nursing. Method: A systematic literature study. The study includes eight research articles. Results: Several studies show a connection between the nurses workload and time pressure. Other studies also points to the nurses reduced ability to nurse-patient- interactions as a result of insufficient time. Professional expectations and subsequent time pressure may influence nurses self-perception of their practice. Time pressure may also lead to negative work strategies. Conclusion: Nurses may experience time pressure that affects quality and safety. Especially through insufficient time for nurse-patient-interactions, occupational health, and negative strategies resulting in consequences for individualisation and possibly nurses observing- and assessment skills. Documentation- and paperwork are described as burdensome for the nurses time, but yet essential for quality and patient safety. A supportive work environment is important for nurses in managing time pressure. Keywords: Time pressure, nurse, quality, patient safety, occupational health.
