Abstract

Title: How do Critical Care nurses engage with children and adolescent that visit the Intensive Care Unit (ICU)? Children and Adolescents` conditions for social participation in an ICU. Master Thesis in Community Work, Bergen University College, Tone Johnsgaard Objective: Community work involves examining opportunities and influencing conditions for social participation. Many intensive care patients have relations to children and/or adolescents that are of great significance to them. The study examined opportunities for social participation by children and adolescents when persons of significance are in an ICU. Method: Qualitative, hermeneutic study, where the empirical material collected from memory work, narrative and focus groups offer indirect access to explore factors inhibiting and enchancing opportunities for participation. A hermeneutic approach, interpreting the text in a dialogue with the text, questions were posed to the text and led to synthesized themes. Results: Four themes were identified; a) Coming for a visit? b) The first meeting c) Prescence on the ICU, and d) Friends – a new significant other? Opportunities for participation of individuals depend on openings to talk, being there for dialogue, getting aquainted and being able to attend to the individuals concerns. Trust, preparation and involvement are important relational conditions. Structural conditions include opportunity to move around within the ward, access to the patient room, and/or a sheltered family room. Conclusion and further research: The findings from this study show that ICU nurses encourage participation, particularly when parents or siblings are patients. When adolescents are admitted, friends often visit on their own initiative. The studys new important contribution to community work is how “the new related”, meaning friends, want to participate through their prescence. Further research should examine children and adolescent` perspectives through their own voice, to understand further factors that inhibit or promote opportunities for participation in the ICU

Bakgrunn/Hensikt: Samfunnsarbeid innebærer å undersøke og påvirke mulighetsbetingelser for sosial deltakelse. Mange pasienter innlagt i en intensivavdeling har nære relasjoner til barn og unge. Dette prosjektets hensikt var å undersøke mulighetsbetingelser for barn og unges sosiale deltakelse som nærstående på en intensivavdeling. Metode: Kvalitativ, hermeneutisk studie der empiri fra minnearbeid, fortellinger og fokusgrupper gir indirekte tilgang til å utforske hemmende og fremmende muligheter for deltakelse. Materialet er analysert ved en hermeneutisk tilnærming, der empiri er tolket som dialog med tekst, og syntetisert i tema. Resultater og diskusjon: Funnene kan samles under temaene a) Å komme på besøk? b) Det første møte c) Barn og unges tilstedeværelse på intensiv, og d) Venner, - de nye nærstående? Mulighetsbetingelser for deltakelse handler om tid og mulighet til å komme, samtale og bli kjent, og forklare ting den enkelte lurer på. Derfor vektlegges relasjonelle aspekter som tillit, forberedelse og involvering. Strukturelle betingelser inkluderer mulighet til å bevege seg i avdelingen, gå til og fra rommet, eller være på pårørenderommet. Konklusjon og videre forskning: Studien viser at sykepleierne oppfordrer til deltakelse, spesielt når foreldre eller søsken er pasient. Når ungdom innlegges kommer jevnaldrende venner på eget initiativ. Studiens viktigste bidrag til samfunnsarbeid er “de nye nærstående”, altså venner som ønsker å delta ved at de kommer til avdelingen. Videre forskning bør se direkte på barn og unges perspektiver for å forstå hvilke faktorer som hemmer eller fremmer deres mulighet for deltakelse på denne arenaen som nærstående.
