Velmeinande overtaling – eller tvang?

Abstract

Bakgrunn for val av tema: Mi erfaring frå praksis, var at tvang vart mykje brukt i den daglege omsorga til pasientar som lever med demens i sjukeheim. Eg ynskjer å finne ut meir om kva type tvang som vert nytta, under kva forhold og korleis sjukepleiar kan bidra til redusera tvangsbruken. Problemstilling: Korleis kan sjukepleiar ivareta autonomien til personar som lever med demens i sjukeheim og redusera bruken av tvang gjennom omsorg? Hensikt: Hensikt med oppgåva var å få meir inngåande kunnskap om korleis sjukepleiar kan ivareta verdigheit og autonomi til pasientar som lever med demens, og kva alternative metodar ein kan nytte for å unngå tvang. Metode: Oppgåva er ein litteraturstudie, der søk i databasane Cinahl og Svemed+, i tillegg til manuelle søk ført fram til valte artiklar. Seks artiklar, i tillegg til omsorgsteori frå Kari Martinsen og den personsentrerte omsorgsteorien frå McCormack & McCance dannar grunnlaget for drøftinga, saman med eiga erfaring frå praksis. Funn: Forsking viser at pasientar som lever med demens i sjukeheim vert utsett for tvang i den daglege omsorga. Det går fram av artiklane at for lite personale, for få personale med relevant kompetanse innan demensomsorg og mange arbeidsoppgåver som ikkje er pasientrelatert gjer at personalet ikkje har tid til å prøve alternative tiltak til tvang. Kontekstuelle forhold som pleiekultur, pasientgruppe og forhold til leiinga har og innverknad på bruken av tvang. Nøkkelord:Tvang, demens, personsentrert omsorg, autonomi, verdigheit og sjukeheim
