Abstract

Problemstilling: Hvordan kan vi som sykepleiere i hjemmebasert omsorg sørge for best mulig oppfølging av pasienter som lider av schizofreni? Bakgrunn: Vi har erfaring fra psykisk helsearbeid innenfor flere ulike instanser, og synes dette er et svært interessant fagområde. Det vi har erfart er at det ofte kan være dårlig oppfølging av pasienter med psykiske lidelser i eget hjem. Vi ønsker derfor å tilegne oss mer kunnskap om hvordan vi som sykepleiere kan sørge for god oppfølging til denne pasientgruppen. Hensikt: Å få økt kunnskap om hvordan sykepleiere kan gi god oppfølgingen til hjemmeboende pasienter som lider av schizofreni, og hvordan dette fungere i praksis. Metode: Litteraturstudium Sammendrag: Denne oppgaven belyser viktigheten av kontinuitet og god samhandling mellom alle involverte parter, for å kunne sikre god oppfølging til hjemmeboende pasienter som lider av schizofreni. Vi har valgt å rette fokus mot hva litteratur, forskning, og lovverk sier, i forhold til oppfølgingen av pasienter med psykiske lidelser. Videre blir dette drøftet opp imot egne erfaringer fra praksisfeltet.
