Dolor crónico y estrategias de afrontamiento

Abstract

El presente artículo analiza el amplio cuerpo de literatura existente sobre el afrontamiento y la adaptación al dolor crónico. A nivel general, aquellas estrategias que en los diversos estudios han mostrado ser más eficaces con respecto al dolor crónico son las estrategias de afrontamiento activas, las centradas en el problema, focalizadas en el dolor y conductuales. Las estrategias concretas sobre las que existe evidencia más concluyente son distracción de la atención, catastrofismo y fantasías agradables como estrategias relacionadas con un mayor dolor e incapacidad. Por otro lado , las auto-afirmaciones y el control percibido del dolor destacan como estrategias adaptativas. En definitiva, la revisión pone de manifiesto la necesidad de continuar con esta línea de investigación, desarrollando instrumentos de evaluación en los que el afrontamiento no sea confundido con otros constructos como creencias o funcionamiento. Por último, se sugiere la realización de estudios longitudinales para conocer los efectos a largo plazo de determinadas estrategias y el papel que juegan considerando la adaptación al dolor crónico como un proceso dinámico.
