El trauma oclusal como factor de riesgo para las retracciones gingivales palatinas y linguales en pacientes fumadores

Abstract

El hábito de fumar es considerado un factor de riesgo para la prevalencia, severidad y extensión de las patologías periodontales. La literatura menciona que los pacientes fumadores exhiben más retracciones gingivales en general, y palatinas y linguales en particular que los pacientes no fumadores. Para este aspecto, no hay trabajos que expliquen los posibles mecanismos biológicos que justifiquen su ocurrencia. La observación clínica de pacientes con estas características, permiten advertir que existiría un vinculo entre las retracciones en general, y las palatinas/linguales en particular, con las relaciones oclusales traumáticas. Se trabajo con 19 pacientes (10 fumadores y 9 no fumadores) con retracciones palatinas y/o linguales a los que se les realizó terapia periodontal basica, radiografías estandarizadas pre y post tratamiento, y el contro de las relaciones oclusales traumaticas a través del uso de una placa de relajación. Al finalizar el estudio, la totalidad de los pacientes mejoraron en la posición del margen gingival y en los demás parámetros analizados. Se cocluye en que el trama oclusal tendría inferencia en las retracciones ya que, neutraliza su acción, ambos grupos de pacientes respondieron de manera similar a la terapia implementada.
