Ajuste cronobiológico del tratamiento farmacológico antihipertensivo.

Abstract

El estudio recientemente publicado por Hermida et al. ha puesto de manifiesto la trascendencia del tratamiento nocturno para la hipertensión arterial (HTA), sin que se hayan encontrado diferencias en cuanto a eventos adversos o efectos secundarios. Nuestro estudio sobre el ajuste cronobiológico del tratamiento antihipertensivo pretende conocer el efecto sobre la tensión arterial tras el cambio de tratamiento de matutino a nocturno, sin modificación de dosis total diaria. Se seleccionará a la totalidad de los pacientes de 40 a 64 años de un cupo médico de Atención Primaria, con diagnóstico previo de HTA y que tomaban al menos 1 fármaco antihipertensivo por la mañana. Se recogerán las características clínicas que de acuerdo con las guías de práctica clínica son trascendentes en los pacientes hipertensos. A todos los pacientes se le medirá la tensión arteri al en consulta de manera tradicional, previo a su inclusión en el estudio, para excluir a aquellos pacientes con una HTA grado II o superior. Se les realizará una monitorización ambulatoria de presión arterial (MAPA) durante 48 horas previamente al cambio de posología y posteriormente a los 3 meses del cambio de tratamiento. Tras el último MAPA se volverá a tomar la tensión arterial en consulta de manera tradicional. Se recogerán todos los efectos adversos y secundarios que sucedan durante el estudio. Se realizará el análisis descriptivo general, análisis estadístico para ver si existen o no diferencias en control de la HTA y por último un análisis estratificado por características clínicas para evaluar posibles sesgos.
