Efecto del boxeo en la encefalopatía crónica postraumática

Abstract

La encefalopatía crónica postraumática (ECP) se trata de una enfermedad neurodegenerativa que tradicionalmente se ha asociado al boxeo debido a la gran cantidad de traumatismos craneoencefálicos a los que se encuentran expuestos. Este trabajo realiza una revisión bibliográfica sobre la relación entre el boxeo y la ECP, sus mecanismos fisiopatológicos, características neuropatológicas, diagnóstico y posibles estrategias terapéuticas. La encefalopatía crónica postraumática se caracteriza por cambios macroscópicas como la atrofia cerebral y cavum septum pellucidum y cambios microscópicos como la acumulación anómala de proteínas tau y TDP-43, neuroinflamación y degeneración axonal. El diagnóstico de la encefalopatía crónica postraumática ha avanzado mediante neuroimagen (RM, PET y SPECT) y la búsqueda de biomarcadores en el líquido cefalorraquídeo y la sangre, aunque actualmente sigue presentado limitaciones. En cuanto al tratamiento a día de hoy no disponemos de terapias curativas y los enfoques se centran en el manejo sintomático. Se destaca la importancia de identificar factores genéticos como el gen de la apolipoproteína E (APOE) siendo el alelo ε4 el importante, ya que su presencia podría predisponer a padecer la enfermedad. Este estudio resalta la necesidad de más investigación para mejorar el diagnóstico precoz, establecer biomarcadores específicos y desarrollar estrategias preventivas y terapéuticas eficaces.
