Abstract

Introducción: La artrodesis tibio-talo-calcánea es un procedimiento de salvamento para pacientes con artrosis severa, desaxaciones o patología neuropática, entre otras. Debido a los beneficios que ofrece, la artroscopia posterior se ha antepuesto a los abordajes abiertos. La reducción del número de complicaciones, del tiempo de hospitalización, del tiempo de consolidación radiológica y del dolor postquirúrgico son algunas de sus ventajas. Un resultado exitoso no depende solo del tipo de abordaje, sino también del método de fijación elegido, pudiendo realizarse este mediante placas, tornillos, clavos y fijadores externos. Objetivo: Describir los resultados demográficos, clínicos, funcionales y radiológicos de los pacientes intervenidos de artrodesis tibio-talo-calcánea asistida vía artroscopia posterior en nuestro centro. Material y métodos: Se realiza estudio descriptivo retrospectivo compuesto por 10 pacientes intervenidos entre enero de 2018 y diciembre de 2023 con un seguimiento postquirúrgico de al menos un año. Se revisan historias clínicas y se recogen variables demográficas, clínicas y radiológicas. Resultados: En nuestra serie no se encontraron complicaciones postquirúrgicas, ni pseudoartrosis. El tiempo medio de consolidación radiológica fue 13’4 +5’7 semanas. La mediana del tiempo de hospitalización es de 2 días. En la Escala Visual Analógica del dolor pre y postquirúrgico, existen diferencias estadísticamente significativas, siendo el valor medio previo a la cirugía de 8’3+1’2 y tras 6 meses de la cirugía de 2’6+2’5 puntos. La mediana del score AOFAS de tobillo y retropié a las 24 semanas de la cirugía fue de 73 puntos. Conclusión: La artrodesis tibio-talo-calcánea asistida vía artroscopia posterior es un abordaje favorecedor para los pacientes, principalmente para aquellos que presentan comorbilidades asociadas. La disminución en el número de complicaciones, así como en el tiempo de hospitalización hacen que sean cada vez más los cirujanos que optan por ella. Además, al reducir el tiempo de consolidación radiológica se consigue una recuperación funcional precoz.

Introduction: Tibiotalocalcaneal arthrodesis is a salvage procedure for patients with severe osteoarthritis, deaxation or neuropathic pathology, among others. Due to the benefits it offers, posterior arthroscopy has taken precedence over open approaches. The reduction in the number of complications, hospitalization time, radiological consolidation time and postoperative pain are some of its advantages. A successful result depends not only on the type of approach, but also on the method of fixation chosen, which can be by plates, screws, pins and external fixators. Objective: To describe the demographic, clinical, functional and radiological results of patients undergoing posterior arthroscopic-assisted tibiotalocalcaneal arthrodesis at our center. Material and methods: We performed a retrospec8ve descriptive study composed of 10 patients who went under surgery between January 2018 and December 2023 with a post-surgical follow-up of at least one year. Medical records were reviewed and demographic, clinical and radiological variables were collected. Results: No postoperative complications or pseudarthrosis were found in our series. The mean radiological consolidation time was 13.4 +5.7 weeks. The median hospitalization time was 2 days. In the Visual Analogical Pain Scale for pre and postsurgical pain, there were statistically significant differences, with a mean value of 8.3+1.2 before surgery and 2.6+2.5 points 6 months afer surgery. The median AOFAS ankle and hindfoot score 24 weeks afer surgery was 73 points. Conclusion: Posterior arthroscopically assisted tibiotalocalcaneal arthrodesis is a favorable approach for patients, especially for those with associated comorbidities. The decrease in the number of complications, as well as in hospitalization time, means that more and more surgeons are opting for it. In addition, by reducing the radiological consolidation time, early functional recovery is achieved.
