Abstract

En la sociedad actual, la pérdida perinatal sigue siendo un tema tabú, así como los duelos y trastornos psicopatológicos derivados de las mismas, como es el trastorno de estrés postraumático (TEPT). La terapia por desensibilización y reprocesamiento por movimientos oculares (EMDR) es una de las técnicas centradas en trauma que ha mostrado ser efectiva para el TEPT. El propósito de este caso único es la realización de un tratamiento psicológico en una clínica de psicología privada con una mujer de 38 años que presenta sintomatología postraumática crónica desde la muerte intrauterina de su primera hija hace 14 años. Para ello se realiza una evaluación inicial y otra final, y un tratamiento con terapia EMDR compuesto por 15 sesiones semanales de cincuenta minutos. Se realizó una entrevista informativa inicial, seguida de una evaluación posterior mediante la Entrevista Diagnóstica para el TDAH en Adultos (DIVA 2.0), el Test de los 90 Síntomas (SCL-90-R), el Inventario de Depresión de Beck (BDI), el inventario Inventario de Evaluación de la Personalidad (PAI), el Inventario del TEPT según el DSM-5 (PCL-5) y la Escala de Experiencias Disociativas (DES). Los resultados finales tanto en el PCL-5 como en las mediciones finales del grado de perturbación asociado a las memorias traumáticas y validez de la cognición positiva instaurada en las mismas, sugieren que la intervención fue satisfactoria y tuvo como resultado la correcta integración de las memorias traumáticas de la paciente con su consiguiente resultado negativo en TEPT.

Today, perinatal loss continues to be a taboo topic, as well as grief and psychopathological disorders derived from it, such as post-traumatic stress disorder (PTSD). Eye movement desensitization and reprocessing therapy (EMDR) is one of the trauma-focused techniques that has been shown to be effective for PTSD. The purpose of this unique case is to carry out a psychological treatment in a private psychology clinic with a 38-year-old woman who presents chronic post-traumatic symptoms since the intrauterine death of her first daughter 14 years ago. To this end, an initial and final evaluation is carried out, and a 15 weekly sessions of fifty minutes treatment with EMDR therapy. An initial informational interview was conducted, followed by a subsequent assessment using the Diagnostic Interview for ADHD in Adults (DIVA 2.0), the 90 Symptom Test (SCL-90-R), the Beck Depression Inventory (BDI), the Personality Assessment Inventory (PAI), the DSM-5 PTSD Inventory (PCL-5), and the Dissociative Experiences Scale (DES). Final results of both the PCL-5 and the final measurements of the degree of disturbance associated with traumatic memories and the validity of the positive cognition established in them, suggest that the intervention was satisfactory and resulted in the correct integration of the patient's traumatic memories with its consequent negative result in PTSD.
