Abstract

El tofo gotoso es una complicación de la gota no controlada que se caracteriza por depósitos de ácido úrico en las articulaciones, causando dolor, inflamación, deformidad y limitación de la movilidad. Nuestro objetivo es realizar una revisión bibliográfica de la efectividad y seguridad de diversas técnicas quirúrgicas para tratar el tofo gotoso en el pie en pacientes de 45 años o más. Material y métodos: Se llevó a cabo una revisión bibliográfica en bases de datos como PubMed, Scopus y Google académico. Se seleccionaron estudios con calidad metodológica y relevancia temática. Resultados y discusión: Hemos identificado diversas técnicas quirúrgicas para el tratamiento del tofo gotoso en el pie, cada una con sus propias ventajas y desventajas. La elección de técnica depende de factores como el tamaño y la ubicación del tofo, la gravedad de los síntomas y el estado general del paciente. Las técnicas comunes incluyen escisión simple, curetaje, osteotomía y artroplastia. Conclusión: El tratamiento quirúrgico del tofo gotoso en el pie es efectivo para controlar los síntomas y mejorar la calidad de vida. La elección de la técnica adecuada debe basarse en una evaluación individualizada, considerando las características del paciente y la gravedad de la enfermedad. Esta revisión proporciona evidencia científica actualizada sobre las diferentes técnicas quirúrgicas, lo que puede contribuir a mejorar la toma de decisiones clínicas en este ámbito.

Gouty tophus is a complication of uncontrolled gout that is characterized by deposits of uric acid in the joints, especially the foot, causing pain, inflammation, deformity and limitation of mobility. The objective of this work is to carry out a bibliographic review of the effectiveness and safety of various surgical techniques to treat gouty tophus in the foot in patients aged 45 years or older. Material and methods: A bibliographic review was carried out in databases such as PubMed, Scopus and Google academic. Studies with methodological quality and thematic relevance were selected. Results and discussion: We have identified various surgical techniques for the treatment of gouty tophus in the foot, each with its own advantages and disadvantages. The choice of technique depends on factors such as the size and location of the tophi, the severity of the symptoms, and the patient's general condition. Common techniques include simple excision, curettage, osteotomy, and arthroplasty. Conclusion: Surgical treatment of gouty tophus in the foot is effective in controlling symptoms and improving quality of life. The choice of the appropriate technique should be based on an individualized evaluation, considering the characteristics of the patient and the severity of the disease. This review provides updated scientific evidence on the different surgical techniques, which can contribute to improving clinical decision-making in this area.
