Consecuencias motoras en la tendinopatía del manguito rotador

Abstract

Introducción: La tendinopatía del manguito rotador se produce por irritación de los tendones e inflamación de la bursa, produciendo dolor y limitación al movimiento. Además, produce alteraciones motoras. Pero, estas alteraciones ¿son debidas a la sintomatología dolorosa? Conocer la fuente de los cambios motores que se producen en la patología podría abrir un nuevo campo de posibilidades terapéuticas y orientar los tratamientos actuales. Objetivo: Estudiar los cambios motores producidos en pacientes con tendinopatía del manguito rotador para verificar si se asemejan a los efectos motores del dolor inducido experimentalmente. Material y método: Se realizó una búsqueda bibliográfica en las bases de datos Pubmed, Science Direct, PEDro, Scopus y Cochrane de los artículos que recogieran los cambios motores que la patología produce. Resultados: Se analizaron un total de 12 ensayos realizados sobre sujetos sanos y patológicos. Los principales resultados observados en sujetos con patología fueron la disminución del tono muscular durante una contracción máxima, así como una disminución en la excitabilidad corticoespinal del músculo doloroso. En cambio, se mantuvo la funcionalidad en la realización de tareas, pero con una cinemática escapular alterada como consecuencia de una adaptación del sistema motor. Conclusión: El dolor inducido y el producido por la tendinopatía del manguito rotador producen alteraciones motoras parecidas tanto en la actividad muscular como en la activación del cerebro y en la cinemática del movimiento.

Introduction: Rotator cuff tendinopathy is caused by irritation of the tendons and inflammation of the bursa producing pain and limitation of movement. In addition, it produces motor alterations. But are these alterations due to the painful symptomatology? Knowing the source of the motor changes could allow therapeutic possibilities and guide current treatments. Objective: To study the changes that pain produces on patients with rotator cuff tendinopathy, to verify if they resemble the motor effects of experimentally induced pain. Material and method: A bibliographic search was carried out in the Pubmed, Science Direct, PEDro, Scopus and Cochrane databases for articles on the motor changes produced by the pathology. Results: A total of 12 trials on healthy and pathological subjects were analyzed. The main findings observed in subjects with pathology were a decrease in muscle recruitment during maximal contraction as well as a decrease in corticospinal excitability of the painful muscle. In contrast, functionality was maintained in the performance of tasks but with altered scapular kinematics as a consequence of an adaptation of the motor system. Conclusion: Experimentally induced pain and that produced by rotator cuff tendinopathy produce similar motor alterations in muscle tone, brain activation and movement kinematics.
