Abstract

Chronic Kidney Disease (CKD) is defined as a heterogeneous disorders affecting kidney structure and function. CKD is an epidemic which affects more than 10% of the population in many countries worldwide. Some four million persons in Spain suffer this disease, and the hemodialysis is the main renal replacement therapy in this patients. In this paper, it has been made an analysis of the renal insufficiency and it focuses on the hemodialysis as the main therapy. In addition, a patient assessment is done according to his basic needs and the standardized care plan is established. The aim of this paper is the patient care planning for people in the hospital hemodialysis units using the NANDA, NOC and NIC´s taxonomies. Standardized care plans allow the provision of similar nursing cares and it also improves quality and care continuity in nurse´s working.
