La artritis reumatoide es una enfermedad crónica, inflamatoria y autoinmune. Afecta a la membrana sinovial de las articulaciones causando dolor, inflamación, rigidez, deformidad y finalmente su destrucción, provocando disminución funcional e incluso discapacidad de quien la padece. No se puede prevenir y su etiología es desconocida, aunque hay cierta predisposición genética y factores ambientales externos e internos podrían contribuir como desencadenantes de la enfermedad. La investigación ha permitido identificar posibles dianas terapéuticas como el TNF, la IL-1 o la IL-6. El conocimiento reciente sobre la genética y la estructura de las inmunoglobulinas, junto al desarrollo de técnicas de cultivo celular y manipulación genética, ha conducido a la producción de anticuerpos monoclonales con potencial terapéutico sobre la AR. Esta inmunoterapia en constante revisión y estudio, ha mostrado eficacia, creando nuevas esperanzas para los enfermos pues frena la progresión de la enfermedad, mejorando su funcionalidad y calidad de vida. Es necesario informar y formar al personal enfermero ya los pacientes en esta nueva estrategia terapéutica, llevando a cabo un completo y correcto seguimiento de la enfermedad y de los posibles efectos adversos.
Rheumatoid arthritis is a chronic autoimmune inflammatory disease. It affects the joints´ synovial membrane causing pain, swelling, inflammation stiffness, deformation and finally their destruction, leading to reduced functionality, even long-term disability. It cannot be prevented and its etiology is unknown, but certain genetic predisposition and outer and inner environmental factors could contribute as triggers of the disease. Research has allowed to identify potential therapeutic targets such as TNF, IL-1 or IL-6. The recent understanding about gene sequences and structure of immunoglobulins, besides to the development of cell culture and genetic manipulation has led to monoclonal antibodies production with potential therapeutic power in RA. Immunotherapy, which is steadily under review and study, has shown efficacy, breathing new hope for RA patients, given that it mitigates disease progression, improving their functionality and quality of life. It is necessary to inform and educate both nursing staff and patients about this new therapeutic strategy, together with carrying out a complete and fit monitoring the disease and the possible side effects.
