Abstract

RESUMEN: El trasplante de páncreas es un tratamiento poco común, en parte debido a los malos resultados iniciales. Por este motivo, la selección de los donantes suele ser rigurosa. Para evaluar el impacto de la calidad del donante en la supervivencia del injerto se han elaborado índices pronósticos, a destacar el Pancreas Donor Risk Index (PDRI). El objetivo principal de este trabajo es analizar la validez del mismo sobre los pacientes trasplantados de páncreas en el HUMV. Se realizó una revisión bibliográfica sobre el estado actual del trasplante de páncreas y sus factores pronósticos. Se recogió de manera retrospectiva información acerca de los donantes de los 59 trasplantes pancreáticos realizados en nuestro centro hasta la fecha, así como datos sobre la supervivencia del injerto y complicaciones. Se realizó un análisis estadístico para comparar los casos con PDRI de alto y bajo riesgo. Se utilizaron variables para comparar la supervivencia del injerto, las complicaciones y la función del mismo. Como resultados del estudio no pudo demostrarse la utilidad del PDRI como predictor de complicaciones o supervivencia del injerto pancreático en nuestra serie, si bien la falta de significación estadística podría deberse a un insuficiente tamaño muestral. Es lógico pensar que la calidad del donante influye en el pronóstico del trasplante pancreático, aunque según nuestros resultados otros factores podrían ser más importantes a la hora de predecir el fracaso del trasplante pancreático a corto y medio plazo.

ABSTRACT: In today’s clinical practice, pancreas transplantation is not a widespread treatment, due to the fact that results with this technique were not so encouraging at the beginning. This is the reason why donor selection is so strict. In order to assess the impact of the pancreas donors’ quality in graft survival, many prognosis indicators have been developed, being the most relevant the Pancreas Donor Risk Index (PDRI). The aim of this work is to analyze the validity of PDRI in pancreas transplant receptors in our hospital: Marques de Valdecilla University Hospital (HUMV). Through these lines we review the data available related with pancreas transplantation and its prognostic factors. We did a retrospective data collection of the 59 pancreas transplants performed in our center so far, and the graft survival and complications emerged. We statistically analyzed the graft survival, complications and its functions. As results, we could not demonstrate PDRI utility as a complications predictor or as a graft survival predictor in our case series, however, this could be caused by the limited sample size. It is reasonable to think that the donors’ quality plays a role in the pancreas transplantation prognosis although according to our results some other factors could be more relevant in order to predict the short-term and long-term pancreas transplantation failure and its complications.
