Abstract

La amnesia global transitoria es un síndrome clínico caracterizado por la aparición brusca de amnesia anterógrada acompañada de preguntas reiterativas, que en determinadas ocasiones asocia un componente retrógrado, de duración inferior a 24 horas, sin compromiso de las funciones neurológicas. Aunque hace más de 50 años desde que se describió por primera vez, la etiología de este síndrome permanece aún desconocida, siendo uno de los síndromes más enigmáticos de la neurología clínica. Existen diferentes teorías acerca de su fisiopatología que la relacionan con factores como la migraña, isquemia focal, anormalidades del flujo venoso cerebral, y fenómenos epilépticos, pero ninguna explica consistentemente los casos de AGT. Es un diagnóstico clínico basado en la obtención de una detalla historia que incluya la información aportada por los testigos del episodio. Las pruebas complementarias son estrictamente normales, residiendo su valor principal en la exclusión de diagnósticos alternativos causantes de amnesia transitoria potencialmente graves o recurrentes como accidentes cerebro vasculares, epilepsia, hipoglucemia, intoxicaciones o amnesia postraumática. No existe un tratamiento específico para la AGT, la recuperación generalmente se produce en las primeras 24 horas tras el episodio y no se ha demostrado ninguna relación con la aparición posterior de ictus. Por el contrario existen estudios que evidencian un mayor riesgo a largo plazo de recurrencia, deterioro cognitivo y epilepsia de nueva aparición.

Transient global amnesia is a clinical syndrome characterized by the sudden onset of anterograde amnesia accompanied by repetitive questioning, sometimes with a retrograde component, lasting up to 24 hours, without compromise of other neurologic functions. Though it is more than 50 years since it was first described, the etiology of this syndrome remains still unknown, being one of the most enigmatic syndromes in clinical neurology. Although various factors such as migraine, focal isquemia , venous cerebral flow abnormalities, and epileptic phenomena have been suggested to be involved in the physiopathology of TGA, none of them explains consistently the cases of TGA. It is a clinical diagnosis based on a detailed history that includes the information reported by the witnesses of the episode. Additional tests are strictly normal, their main value is to exclude alternative severe or potential recurrent diagnoses of amnesia such as cerebral isquemia, post-traumatic amnesia, epilepsy, hypoglycemia or intoxication. It does not exist a specific treatment for TGA, improvement is noted within 24 hours without any intervention and no relation has been demonstrated with stroke. However, there are a few studies on the long-term outcome of patients who have experienced TGA regarding the risk of recurrence, cognitive decline and seizures over time. .
