Abstract

La hipótesis conceptual: Se apoya en la demostración de que un aspecto muy importante en la cascada metastásica es el proceso de degradación de la matriz extracelular por enzimas proteolíticas específicas. Las células tumorales tienen capacidad de juntar sus mecanismos proteolíticos con la motilidad de una forma altamente organizada para conseguir invasión. Un papel fundamental en este proceso de degradación se atribuye a las metaloproteinasas de matriz. La relación entre la expresión de estas proteínas y el pronóstico en otros tumores ya se ha documentado. La hipótesis operativa del trabajo: Se centra en considerar que la detección de la expresión de la MMP-11 y TIMP-2 en el tejido tumoral puede establecer de manera no invasiva grupos de diferente pronóstico en pacientes con carcinoma de pulmón y servir de base para futuros esquemas terapéuticos. Metodología: Estudio retrospectivo de expedientes de 44 pacientes tratados quirúrgicamente por adenocarcinoma pulmonar con más de 5 años de seguimiento estableciendo dos grupos en función de la evolución (favorable y no favorable) tras el tratamiento. En estos pacientes se realiza un estudio de la expresión en la matriz tumoral de la Metaloproteinasa 11 y del inhibidor TIMP-2 estudiando la relación existente entre la expresión de ambas con la capacidad invasiva del tumor, las características clínico-patológicas de los pacientes el desarrollo de metástasis a distancia y finalmente con la supervivencia.
