Abstract

La melatonina es una neurohormona secretada principalmente por la glándula pineal. Si bien clásicamente ha sido conocida por su papel en la sincronización de los ritmos circadianos del organismo con la luz ambiental, además ejerce una gran variedad de acciones, entre las que destaca su efecto antitumoral, participando en todas las etapas del desarrollo del cáncer: iniciación, progresión y metástasis. Entre los mecanismos antitumorales de la melatonina están sus acciones antiestrogénica, antioxidante y antiangiogénica, su papel en la regulación del ciclo celular y la inmunidad, su capacidad para inhibir la actividad de la telomerasa, su actuación en la prevención de la disrupción del ritmo circadiano y su regulación de los cambios epigenéticos y del transporte y metabolismo de los ácidos grasos. Este efecto antitumoral se ha estudiado tanto in vitro como in vivo en distintos modelos animales y en humanos, y sobre distintos tipos de tumores, siendo especialmente notable en los cánceres hormonodependientes, como los de mama y próstata. En los últimos años se ha demostrado la capacidad de la melatonina de actuar como adyuvante a la quimioterapia y radioterapia, ya que es capaz de aumentar su eficacia y reducir los efectos secundarios de estos tratamientos.

Melatonin is a neurohormone mainly secreted by the pineal gland. While it has long been known for its role in regulating the body's circadian rhythms in response to ambient light, it actually exerts a wide range of actions, notably its antitumor effect, participating in all stages of cancer development, from initiation and progression to metastasis. Among melatonin antitumor mechanisms are its anti-estrogenic, antioxidant and anti-angiogenic action, its role in regulating the cell cycle and immunity, its ability to inhibit telomerase activity, its involvement in preventing circadian rhythm disruption, its regulation of epigenetic changes and its influence on fatty acids transport and metabolism. This antitumor effect has been studied both in vitro and in vivo in different animals and in humans, across different types of tumors, with particularly notable effect in hormone dependent cancers, such as breast and prostate cancer. In recent years, melatonin has been shown to act as an adjuvant to chemotherapy and radiotherapy, as it can enhance their effectiveness and reduce the side effects of these treatments.
