Abstract

El empleo del CGRP como biomarcador de la migraña es un tema controvertido ya que los trabajos sobre esta temática han arrojado conclusiones en muchos casos contradictorias. El origen de estas discrepancias es por el momento desconocido, aunque la evidencia parece indicar que existen una serie de parámetros metodológicos no analizados que de algún modo determinan los resultados. Partiendo de esta hipótesis, desarrollamos una metodología de medición lo más precisa y detallada posible, atendiendo a aquellas variables que se han identificado como sensibles de alterar las determinaciones del péptido, de tal manera que se asegurasen unos resultados robustos y reproducibles. Con este protocolo se pudo testar tanto la especificidad como la sensibilidad de los niveles de CGRP para la migraña crónica. Observamos que una elevación del CGRP-alfa era un evento sensible para esta entidad, aunque puedan existir otras enfermedades, tal y como describimos en el COVID- 19, en las que también se presenten concentraciones alteradas, restando así especificidad del evento por la migraña crónica.

The use of CGRP as a biomarker for migraine is a controversial topic, as studies on this subject have often produced contradictory conclusions. The origin of these discrepancies is currently unknown, although the evidence seems to suggest that there are a number of unexamined methodological parameters that somehow determine the results. Based on this hypothesis, we developed a precise and detailed measurement methodology, taking into account those variables identified as potentially altering the peptide measurements. This approach ensured robust and reproducible results. Using this strategy we could test the specificity and sensitivity of CGRP levels for chronic migraine. We observed that an elevation of alpha-CGRP was sensitive for this condition, although other diseases, such as COVID-19, may also show altered levels, thereby reducing the specificity of this event for chronic migraine.
