Abstract

Introducción: La infección por Virus del Papiloma Humano y el cáncer de cuello de útero han ido evolucionando a lo largo de los años debido a la instauración de la vacunación universal y el uso de la citología como técnica de cribado. Método: Estudio descriptivo y retrospectivo de 2009 mujeres que se sometieron al cribado oportunista de Cáncer de cérvix del Servicio Cántabro de Salud en las fechas comprendidas del 1 de enero hasta el 19 de Octubre de 2022. Se les realizó una citología líquida analizada por PCR e hibridación en array con sondas específicas frente a 35 genotipos de VPH. Se compararon los datos obtenidos con los que habían sido publicados por el HUMV en análisis previos tanto previos a la instauración de la vacunación como en años inmediatamente posteriores. Resultados: Las infecciones causadas por genotipos de bajo riesgo incluidos en la vacuna han disminuido de manera significativa. El VPH31 a pesar de no estar incluido en los genotipos cubiertos, presenta un descenso de la incidencia significativa como consecuencia de la inmunidad cruzada. La incidencia de VPH45 y VPH51 ha descendido, aunque no significativamente. Sin embargo, se observa un aumento significativo de la incidencia de VPH52 entre la población previa a la vacunación y la actual posiblemente explicada por un efecto nicho. El VPH16 al que se atribuye mayor poder oncogénico ha descendido de manera significativa en toda la población. Conclusión: El uso del genotipado como método de cribado del VPH constituye una herramienta fundamental para un mayor conocimiento de los cambios y la biología de la infección del VPH.

Introduction: Human Papilloma Virus infection has been evolving over the years due to the stablishment of Cervarix universal vaccination and the use of citology as a screening technique for cervical cancer. Methods: Descriptive and retrospective study of 2009 women who underwent opportunistic cervical cancer screening program of Sevicio Cántabro de Salud from 1st January to 19th October of 2022. Patients underwent liquid citlogy analyzed by PCR and array hybridization with 35 specific probes of different VPH genotypes. The data obtain were compared with previous studies published by HUMV. Results: Low-risk genotypes infections have decreased significantly. VPH31 also shows a significant decrease due to the cross-inmunity. The incidence of VPH45 and VPH51 has cecreased, although not significantly. However, there is a significanta increase in the incidence of VPH52 between the pre-vaccination population and the current one, possibly explained by a niche effect. VPH16 corresponds to the genotype that the greatest oncogenic power is attributed. A very significant decrease in tis incidence was observed in the vaccinated population. Conclusion: Genotyping as an screening method is a fundamental tool for a better understanding of the changes and biology of the VPH infection.
