Abstract

RESUMEN : Las células natural killer (NK) tienen un papel esencial en el correcto desarrollo del embarazo. Intervienen en funciones clave de regulación tanto de la invasión del trofoblasto en la pared uterina como en la remodelación de las arterias espirales para que se produzca una placentación adecuada. Las funciones de las células NK en el útero están moduladas por las interacciones entre las moléculas propias del MHC de clase I y sus receptores inhibidores y activadores. Las células NK uterinas (uNK), presentan una citotoxicidad menor que las células NK en sangre periférica, sin embargo, una citotoxicidad alterada o excesiva se ha descrito como causa tanto de abortos de repetición (AR) como de fallos de implantación (FIR). En este trabajo se ha realizado por un lado, una evaluación de la técnica funcional de citotoxicidad de células NK sanguíneas utilizando la línea celular K562 en pacientes con AR (n=94) y FIR (n=37) y por otro se han recogido una serie de parámetros inmunológicos (porcentaje de células NK, presencia de anticuerpos antifosfolípido, anticuerpos antitiroideos, anticuerpos antinucleares y niveles de C3 y C4) y bioquímicos (vitamina D, TSH y T4) en las historias clínicas de estas pacientes, procedentes de la Unidad de Patología Gravídica del Hospital Universitario Marqués de Valdecilla (HUMV). No se han observado diferencias significativas en las variables estudiadas entre los dos grupos. Esto podría sugerir que, a pesar de ser dos entidades clínicamente bien diferenciadas, las células NK en sangre periférica no parecen tener un papel discriminatorio que ayude a diferenciar estas patologías.

ABSTRACT : Natural killer (NK) cells play an essential role in the proper development of pregnancy. They are involved in key functions of regulation both invasion of the trophoblast into the uterine wall and in the remodeling of the spiral arteries so that a successful placentation occurs. The functions of NK cells in the uterus are modulated by interactions between the MHC class I proper molecules and their inhibitory and activating receptors. Uterine NK cells (uNK), have a lower cytotoxicity than peripheral blood NK cells (pNK), however, an altered or excessive cytotoxicity has been described as the cause of both recurrent miscarriages (RM) and implantation failure (RIF). In this work, on the one hand, an evaluation of the functional technique of blood NK cell cytotoxicity using the K562 cell line in patients with RM (n=94) and RIF (n=37) was carried out and, on the other hand, a series of immunological parameters were collected (percentage of NK cells, presence of antiphospholipid antibodies, antithyroid antibodies, antinuclear antibodies and C3 and C4 levels) and biochemical parameters (vitamin D, TSH and T4) from the medical records of these patients, from the Gravidic Pathology Unit of the Marqués de Valdecilla University Hospital (HUMV). No significant differences were observed in the variables studied between the two groups. This could suggest that despite being two clinically well differentiated entities, NK cells in peripheral blood do not seem to have a discriminatory role that helps differentiate these pathologies.
